13 weeks 36 hrs/wk Rotating shifts BLS Job Title: Behavioral Health Nurse Department/Subsidiary: Behavioral Services Job Code: Y0400 Effective Date: March 1st, 2023 JOB SUMMARY: The Behavioral Health Nurse provides assessments, support and care coordination across the behavioral health care continuum for patients struggling with mental illness as well as individuals and families seeking resources or information about treatment options. The Behavioral Health Nurse is an integral member of Behavioral Health Service Line and works collaboratively with the treatment team members and providers in the various inpatient and outpatient programs offered at Mercy. The Behavioral Health Nurse works closely with community partners to ensure successful transition of care and promotes education to the public on Behavioral Health-related topics through various forums. The Behavioral Health Nurse is responsible for carrying out duties as a health coach as well as utilizing skills as a psychiatric nurse in various care areas when needed, including but not limited to the Intensive Outpatient Program and Psych Triage in the Emergency Department. ORGANIZATIONAL SUMMARY A. Reports to: Behavioral Services Clinical Nurse Manager and Director B. Supervises: None C. Key Working Relationships: Behavioral Health Department Team Members, Community Partners, Community Stakeholders, North Iowa NAMI Chapter, Population Care Management Colleagues, Leadership, Outpatient Clinic Staff. D. Primary Customer Groups: Patients and their families as well as members of community seeking behavioral health education, evaluation, resources and/or support. E. Age of Patients Served: Neonatal X Adolescent X Geriatric Pediatric X Adult All Ages ESSENTIAL FUNCTIONS A. Assesses and evaluates individuals' physical, psychosocial, environmental, spiritual, social and education needs. B. Works collaboratively with the Behavioral Health inpatient and outpatient teams to achieve desired patient care outcomes and ensure a smooth transition of care C. Provides, facilitates, and/or implements learning opportunities to meet the educational needs of patients, families or members of the community. D. When required, documents patient information in the electronic/paper medical record in an accurate, timely manner E. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments. F. Effectively communicates pertinent patient changes/conditions to colleagues and to multidisciplinary caregivers. G. Provides assessments, care coordination services and support to various locations within the hospital when questions or concerns arise related to Behavioral Health resources. H. Demonstrates flexibility and a willingness to provide coverage to other areas in the Behavioral Health Service Line, Including the Intensive Outpatient Program and Psychiatric Triage. I. Performs related age specific patient care duties in various environments that reflect educational needs and concern for comfort and safety needs of the patient. J. Provides competent patient care to the above indicated patient populations under the RN direction by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatment. K. Establishes an effective Health Coach/Patient relationship by demonstrating an understanding of professional coaching ethics, behaviors, and values L. Establishes a trusting and supportive relationship with patient to support a safe and respectful coaching environment M. Reviews high risk patient lists for the clinic or specified population - working with the provider, care team, and other to identify high risk patients appropriate for outreach for Health Coaching N. Collaborate with others whom are part of the patients care team across the continuum and completes SBAR hand-off as necessary. O. Work directly with the inpatient and ambulatory population care management teams to help with general knowledge of appropriate engagement health coaching services. P. Proactively acts as patient advocate, responding to and working to resolve patient concerns. Q. Provides ongoing support for new behaviors related to self-management of chronic disease without cultural, religious or racial bias. Maintains a non-judgmental attitude and demeanor. R. Demonstrates the ability to develop and maintain an effective coaching plan with the client. 1. Utilizes SMART goals. (Specific, Measurable, Attainable, Realistic and Timed) to optimize client success. 2. Assists the client to evaluate progress towards reaching previously set goals. 3. Offers different and varied resources for learning. 4. Acknowledges and celebrates with the client the achievement of patient-centered goals. S. Encourages client accountability by positively confronting the client when he/she fails to take mutually agreed upon actions. T. Maintains required department confidentiality and abides by HIPPA regulations. U. Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments. OTHER V. Communicates with and performs duties following the philosophy of Mercy One- North Iowa and the Sisters of Mercy reflecting the organization's Mission, Values, and Guiding Behaviors. W. Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment. X. Performs other responsibilities as requested by supervisor, manager or department head. ADDITIONAL FUNCTIONS A. Participates on shared governance councils, department/housewide committees/work teams as requested B. Acts as a liaison between community members and agencies and Mercy One - North Iowa. C. Participates in the orientation and education to new Behavioral Health staff and nursing students. POSITION QUALIFICATIONS A. Education 1. Current RN licensure in the state of Iowa. 2. Bachelor of Science in Nursing preferred B. Experience 1. Previous clinical experience in a psychiatric treatment setting, acute care preferred 2. Minimum of 5 years clinical experience. B. Special Skills and Competencies: 1. Ability to be non-judgmental regarding living and health related decisions. 2. Ability to work with client and families with diverse opinions and diverse religious and cultural ideals and values. 3. Ability to work autonomously with minimal direction and be directly accountable for practice. 4. Organization, critical thinking skills and excellent communication skills. 5. Ability to work collaboratively with physicians. 6. Completes the following mandatory courses (check all that apply). X Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status. X Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
