Job Summary

Facilitate the admission and/or transfer of patients to Charleston Area Medical Center taking into consideration the patient's physical condition, the admitting physician's requirements, the referring physician's needs and the institution's resources and scheduling capabilities. Triage Helpline calls to appropriate manager. Provide a defined scope of administrative support to patient care staff on evenings, nights, weekends, and holidays.

Responsibilities

* Receive request from physician for transfer of patient to CAMC; determine the availability of requested placement; contact CAMC physician for acceptance of patient; coordinate three-way conversation between referring and CAMC physicians; assign bed; confirm or assist with transport; acquire all pre-registration information; advise referring hospital/physician of required medical information for transfer; confirm arrangement with CAMC physician; notify CAMC physician of patient's arrival.

* Maintain responsibility for taking all Physician Access Line calls and entering information in SMS system in accordance with admitting criteria and call tracking database.

* Document compliance with standards established to monitor referral activities to medical staff and administration.

* Make recommendations to improve referral process.

* Conduct bed management meetings to proactively deal with patient placement issues.

* Receive reports from patient care management staff regarding bed availability and staffing issues.

* Communicate information on hospital occupancy and bed availability to patient care management staff.

* Arrange for internal transfers in accordance with physicians' orders, bed management guidelines, and unit criteria.

* Collaborate with patient care management staff, medical staff, and utilization management personnel to enforce admission criteria and reduce lengths of stay.

* Monitor compliance with internal policies and procedures related to bed utilization.

* Make recommendations to improve patient flow.

* Triage Helpline calls to appropriate manager.

* Provide a defined scope of administrative services for clinical departments on evenings, nights, weekends, and holidays.

Knowledge, Skills & Abilities

Patient Group Knowledge (Only applies to positions with direct patient contact)

The employee must possess/obtain (by the end of the orientation period) and demonstrate the knowledge and skills necessary to provide developmentally appropriate assessment, treatment or care as defined by the department's identified patient ages. Specifically the employee must be able to demonstrate competency in: 1) ability to obtain and interpret information in terms of patient needs; 2) knowledge of growth and development; and 3) understanding of the range of treatment needed by the patients.

Competency Statement

Must demonstrate competency through an initial orientation and ongoing competency validation to independently perform tasks and additional duties as specified in the job description and the unit/department specific competency checklist.

Education

* Associate's Degree (Required) Experience: 3 Years Related Clinical Nursing Experience in Acute Care Substitution: May substitute Formal Nursing Program for the Associate Degree
