We believe in creating an inclusive culture supported by reducing workplace stress and fostering a healthy environment The hours of the position are 4p-7p on weekdays and 11a-7p on weekends for a total of 20 hours per week.Job Summary: You will have the opportunity to partner with the Activity Director by creating, planning and implementing activities to meet the needs of the residents while complying with all state and federal regulations. Common Responsibilities:

• Assists in planning, developing and implementing activity programming designed to enrich the lives of our residents.
• Leads small, medium and large group activities as well as one-to-one activity programs.
• Documents participation and completes assessments as applicable.
• Works with volunteers for the Activity Department.
• Prepares equipment, supplies and space to accommodate activities and cleans up space after conclusion.
• Transports residents to and from activities off the neighborhood as needed. Includes field trips and outings.
• Ensure proper supervision during activities.
• Make independent decisions when circumstances warrant such action
• Must maintain appropriate documentation, charting activity participation, behaviors, and degree of involvement.
• Adhere to facility Policies and Procedures.
• Work with the Director to create a home like environment in the customerâ€'s room which includes pictures, family photos, plants (please follow dementia guide to toxic plants), afghans, CD player, etc.
• Ensure customer and family confidentiality regarding all care concerns and issues.
• Work tactfully with personnel, residents, visitors and the general public
• Create and maintain an atmosphere of warmth, personal interest, and positivity.
• Ensure that activities are occurring as planned, following the monthly activity calendar

• High School Diploma or GED
• Must have basic computer skills
• 1-2 years of Long-Term Care experience preferred but not required
• Ability to push, move, and or lift a minimum of 50 pounds
• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to work with residents based on whatever cognitive level at which they are currently functioning
• Ability to maintain composure in stressful situations
• Must have a flexible attitude and have ability to multitask
