St. James House is accepting applications for a full-time Activity Director. QUALIFICATIONS Preferredtwo years ofexperience in a social or recreational program within the last five years, one of which wasfull-time in a resident activities program in a health care setting; or has completed a training courseapproved by the State.Must be able to communicate well verbally and in writing.RESPONSIBILITIES Restoringself-confidence, community reintegration, resocialization skills, improving physical condition, leisureeducation for residents, one on one programming for the room/bed bound, providing mentalstimulation and sensory stimulation, and develops a plan for each resident that is designed forrestoration of the quality of life experienced by the resident prior to admittance to the health care facility. 1. Completes a Resident Activity Assessment on all residentswithin seven days ofadmission, prior to care plan conference. Makes an introductory visit to residents within 48 hours ofadmission.2. Based on information as a result of the InterestSurvey/Initial Assessment, identifies problems and needs of the resident and develops an activityplan designed to meet the individual needs of the residents.3. Participatesin resident care plan conferences as schedule by the Director of Nursing.4. Isfamiliar with Federal and State regulations regarding activity programming for long-term care facilities andfunctions accordingly. Participates insurvey exits, and assists in responding to the survey, when appropriate.5. Hasa working knowledge of Resident's Rights, and is an advocate for the resident.6. Is responsible for completion of all documentation requiredby the Activity Department, in a timely and professional manner.7. Communicates with all department heads to insure fullimplementation and understandingof the activity program.8. Is responsible for orienting allactivity personnel to the facility's policies and procedures.9. Providestraining, supervision, and evaluation of all activity personnel.10. Keepsconfidential any and all information regarding the resident's personal andclinical records.11. Presentsa minimum of two in-services annually, designed to educate and/or support staffin the purpose and role of the activities program in the facility.12. Evaluatesthe resident's response to the activity plan as indicated.13. Providesassistance to the Resident Council as requested.14. Developsan ongoing program of meaningful activities, designed to meet the needs andinterests of each resident, and is responsible for the implementation ofprograms.15. Keeps acurrent list of precautions noted on all residents available for reference atall times in the activities office.16. Participatesin community oriented projects, and special marketing projects.17. Schedulesall presentation of visiting groups and coordinates interdepartmentalcommunications regarding special events.18. Coordinatesand supervises the activity volunteer program. Recruits, trains, and orientsvolunteers.19. Plansand coordinates the use of Activity funds within the budget, and keeps accuraterecords, money spent, as well as resident input into spending of funds.20. Transportsresidents on community outings.21. Adaptsactivities to meet resident capabilities.22. Is willingto work flexible hours to meet resident needs including some evening andweekends.23. Performscognitive orientation that may be related to impairment of daily livingfunctions, i.e., orient to person, place, or time; reality orientation; memoryrecall long and short term.24. Maintainsconfidentiality, adhering to established policies on confidentiality in regardsto residents, employees, and family members.25. Isaware of fire and disaster plans and is prepared to function accordingly.26. Is awareof procedures directly related to resident care (infection control, recordkeeping and terminology).27. Awareof chain of command, and functions within role as a professional.28. Maintainsacceptable degree of organization of activity supplies, and keeps inventory ofsupplies and usage.29. Usestime management and supervises the time of Activity personnel.30. Coordinatesfund raisers for the Activity Department.31. Producesmonthly newsletter.32. Performs other tasks as assigned.33. Attendsrequired staff in-services, meetings, orientation, and continuing education activitiesas required.34. Is aware ofand adheres to Patient's Bill of Rights and confidentiality of patientinformation including HIPAA regulations.35. Interacts withpatients and family members, co-workers, clinical and ancillary staff in anon-judgmental, supportive and calm manner.36. Is aware ofPatient Abuse Reporting Law.37.
