Job DescriptionDuties & Responsibilities: Plans, organizes and implements Therapeutic Recreation age appropriate programs based on the assessed needs and interests of patient/resident on assigned units, reflective of culture, leisure history, current interest, and present abilities/limitation.
- Escort residents to and from off unit special events and centralized weekly programs.
- Maintain daily attendance records of scheduled activities on assigned unit and off unit central locations (Activity Pro Software).
Assignments may include supervising other staff personnel, Student Interns, and/or volunteers
- Completes all clinical documentation according to departmental policies and procedures, identifying the needs and interest of every patient/resident on assigned units and when assigned coverage of units in the absence of other TR staff. Documentation includes: assessments (initials, annuals, quarterly and significant change), interdisciplinary care plans, 3.0), progress notes (Point Click Care system).
Attends and may conduct centralized and Departmental In - Services meetings.
- Initiates, plans and coordinates special projects, events and Birthday Parties etc. Breakdown and cleanup after events have taken place.
- Organizes and implements community trips. Responsible for maintenance and posting of daily unit activity schedule, monthly central calendars on assigned unit and at resident's bedside reflecting available therapeutic and leisure activities, including age appropriate, seasonal decorations.
Responsible for ordering and maintaining adequate program supplies and equipment, taking into consideration the age appropriateness of such materials. Maintains and submits on a timely basis resident program participation statistics for unit/central programs and special events (Activity Pro Software). Submits all required departmental request, e.g., Audio -visual equipment requests, community trip referral and request, birthday parties, individual resident calendar, etc. on a timely basis May supervise Activity Therapist II, Activity Therapist I, Community Liaison and Rehabilitation Technicians and Volunteers.
- Represent TR Services during Interdisciplinary Care Plan meetings and Patient Committees, completing necessary documentation (ex. MDS 3.0 and/or Care Plans, Progress Notes in the EMR). Provide coverage in the absence of co-workers.
Assist in Performance Improvement Activities. Follow all facility P & P for safety management, pain management and infection control. Endorses any medical condition to peers as per Departmental Endorsement Policy. May supervise college interns pursuing future credentials. Assist management in special task/programs as assigned. May represent department at interdisciplinary meetings or programs. Maybe assigned educational role during TR week or outside organizations. Follow facility policy regarding Outlook e-mail account and Cisco Phone. Check account daily.
- Utilizes laptop/ I pad / IPOD(Music & Memory Program) for 1:1 or group programming when appropriate.
- Responsible for the monitoring, maintaining/cleanliness of the Art & Culinary Room and Music & Movement Room when conducting programs
Minimum QualificationsAssignment Level I 1. A baccalaureate degree from an accredited college including or supplemented by 18 credits in recreation, physical education, dance, fine arts, applied arts (including therapy) and 12 credits in psychology, social psychology, group work, group dynamics and/or related field; or 2. A baccalaureate degree from an accredited college and one year of full-time experience in planning, coordinating and supervising a recreation activities program in a hospital, health care or related facility; or 3. A satisfactory combination of education and/or experience equivalent to 1 or 2 above. However, all candidates must possess a baccalaureate degree. Assignment Level II In addition to meeting the requirements for Assignment Level I, Assignment Level II requires: 1. One year of full-time experience as described in two (2) above. Assignment Level III In addition to meeting the requirements for Assignment Levels I, Assignment Level III requires: 1.
