BASIC PURPOSE :The Admission Coordinator is responsible for accurate and timely screenings of possible candidates for admission to the health care programs provided by The Davis Community at the Porters Neck and Cambridge Village locations including SNF, rehabilitation, AL, memory care services, wellness services and outpatient services. To ensure confidence in the services of The Davis Community and establish a rapport with referral sources, community case managers, hospitals, physicians and other entities referring possible candidates for admissions. Provide seamless transition during the entire admission process rectifying concerns or issues as they arise. MAJOR WORK ACTIVITIES :1. Screen potential residents for admission to appropriate program placement and ability to pay for services. This includes verifying Medicare, Medicaid, commercial insurance, secondary insurance, and PASSRs as needed.2. Manage with the sales/admission process for all service lines and venues by assisting prospective residents, their families and referral sources in the decision making process by understanding their needs and educating them about how the campus' services and programs can meet their needs. This also includes inquiries, tours, and follows up.3. Organize, coordinate and monitor the provision of cost-effective care in the most clinically appropriate setting. Collaborating with resident, family, MD and ICP.4. Manage the admission process from initial contact through admission including ensuring all required paperwork is complete and timely for each move-in. Enter resident information into the EMR.5. Ensure communication between all other departments concerning any resident move-ins, respite stay, or move-outs. Coordinate communication and decisions with Administrators to ensure all potential residents are appropriate for the campus.6. Monitor benchmarks of admission effectiveness and measure that effectiveness through data assessment. This may include tracking admission rates, referral sources, identifying market needs and trends, etc.7. In conjunction with other members of the admission/marketing/sales team, develop and implement strategic marketing and sales plans to meet/exceed budgeted occupancy.8. Participate in and organize campus marketing/sales efforts with other members of the admission/marketing/sales team to include advertising and promotional activities such as trade shows, print, online, and direct mail to meet the objectives of the marketing and sales plan.9. Be willing and able to fill in/assist other team members in times of need.10. Support management/administration decisions through periods of change or stress.11. Perform specific duties and responsibilities as assigned by the Administrators.RequirementsEducation: Associates Degree or applicable experience. Graduate of BA or BS preferred.Licensure/Certification: N/AExperience: At least two years of previous work experience in long term care admissions preferred.An equivalent combination of education and experience may be considered.
