Nursing Home Admissions Concierge

Location: Willow Tree and Care CenterJob Type:Full-time

Wage: $24 to $27 per hr DOE

Position Summary:

We are seeking acompassionate and highly professionalNursing Home Admissions Conciergeto oversee the admissions process at our community. As the first point of contact for potential residents and their families, you will provide a welcoming and informative experience while ensuring a smooth transition into our care. This role requires aconcierge-levelapproach to customer service, focusing onpersonalized attention, proactive communication, and seamless admissions.

Key Responsibilities:

Initial Contact&Assessment:

• Serve as theprimary point of contactfor prospective residents and families, providing information on services, amenities, and costs.
• Conductinitial consultationsvia phone, email, or in person to assess needs and eligibility.
• Schedule andlead engaging community tours, tailoring each visit to the unique needs of the prospective resident.

Admissions Process Management:

• Guide families through the admissions process withclear communication and compassionate support.
• Collect, review, andverify all necessary documentation, including medical records, insurance details, and financials.
• Coordinate with medical and care teams to assess residents' needs and determine the appropriate level of care.
• Coordinate withhospitals and discharge plannersto facilitate asmooth transitionfrom hospital to community.
• Providetransparent explanationsof policies, payment options, and contractual terms.

Marketing&Outreach:

• Work under thedirection of the Family Advisorto support outreach and referral development efforts.
• Build and maintain strongrelationships with referral sources, including hospitals, healthcare providers, and community organizations.
• Participate inmarketing initiativesandcommunity outreachefforts to promote the community and generate new leads.
• Maintain a database of prospective residents andactively follow up on inquiriesto increase admissions.

Concierge-Level Customer Service:

• Offer awarm, empathetic, and personalizedapproach to each family, addressing concerns with professionalism and care.
• Act as a trustedadvisor and advocate, ensuring prospective residents and families feel confident and supported.
• Coordinate with multiple departments to provide aseamless and welcoming move-in experience.

Administrative Duties:

• Maintainaccurate recordsof all admissions inquiries and applications.
• Manage and updatethe community's waiting list.
• Prepare admission paperworkand ensure compliance with all regulatory requirements.

Qualifications&Skills:

• Valid driver's license requiredfor travel to hospitals, referral partners, and community events.
• Exceptional communication and interpersonal skillsto engage with residents, families, and healthcare providers.
• Strong organizational abilitiesto manage multiple admissions at once.
• Knowledge of healthcare regulations and admissions proceduresin a nursing home setting.
• Sales and marketing experiencepreferred, with the ability to effectively present the community's services and benefits.
• Proficiency in computer systemsfor data entry and record-keeping.
• A compassionate, concierge-like approachto guiding residents and families through the transition process.
