Your responsibilities will extend to coordinating care for short-term rehab residents, which includes facilitating crucial care conferences with families and residents. By attending weekly Medicare meetings and conducting appropriate MDS assessments, you'll help to develop personalized care plans that reflect our commitment to empathy and compassion.Additionally, you will set up discharge services to ensure residents leave with the support they need, all while fostering strong relationships through follow-up and effective communication.
