Job Description: Compass Healthcare Consulting & Placement is conducting a search for an Admissions RN for Hospice, RN Registered Nurse making visits to potential Hospice patients for Admissions. Competitive Salary & Excellent Benefits. Qualified candidates must have NYS RN AND NYS Driver's License as well as access to a reliable automobile. Position requires travel to facilities in the following areas: Bronx or Westchester County, NY. Qualified candidates will have prior experience in medical-surgical, community health, and/or hospice, palliative care, oncology nursing, preferred. Will understand the Hospice Admissions process and will be able to communicate to patients and families regarding available Hospice services. Additional opening available for Admissions RN in Brooklyn/Queens/Nassau County region. Admissions RN - Hospice The Admissions Nurse is responsible for responding to and visiting patients who have been referred for admission to Hospice at Home Program. The Admissions Nurse informs the patients, caregivers, and family members of hospice services available with the organization and assesses the patient for admission to services.Responsible for visiting patients who have admitted to the hospital and communicates the patient status with the respective interdisciplinary team.Possess or demonstrate the ability to develop strong relationships with hospitals, physicians, nursing facilities and other referral sources through outreach, education and networking.Provides education to patients and their caregivers about hospice, evaluates for eligibility and works with the interdisciplinary team to develop an initial plan of care utilizing principles of hospice and palliative care.Responsible for generating and responding to referrals from hospitals and other community partners.Work with physician and interdisciplinary team. Willing to travel on weekends. Qualifications: Current nursing license to practice profession in the state where employee is providing service.Current driver's license in state of residence. Possession of registered and insured motor vehicle.(2) years of experience working an acute care setting(1) year of hospice experience, preferred. Education: Graduate from an accredited school of nursing. RN, BSN preferred.
