Reports To: Operations Manager

Job Summary: Pod Health is seeking a highly organized, detail-oriented, and patient-centered Healthcare Admissions Coordinator to join our dynamic care team. This dual-role position focuses on managing patient admissions, follow-up processes, and ensuring seamless patient-provider interactions. The ideal candidate will bring exceptional administrative expertise, a commitment to excellence in patient care, and a proactive approach to team collaboration and operational efficiency.

Key Responsibilities:

  • Post-Visit Review and Documentation Compliance:
  • Analyze provider notes and patient records in the EMR to identify required follow-up actions.
  • Ensure all service enrollments and care updates are processed promptly.
  • Complete, submit, and ensure all required patient documentation compliance with healthcare regulations and company policies.

CRM and Data Management:

  • Create and manage referral opportunities across service lines.
  • Monitor and update patient progress and service outcomes within the CRM system.
  • Maintain accurate and up-to-date patient data, including insurance and demographic information.
  • Verify service eligibility to facilitate care coordination.

Service and Appointment Coordination:

  • Collaborate with internal case managers and service specialists to resolve outstanding tasks.
  • Communicate with stakeholders to address and mitigate service delays.
  • Confirm appointments with patients 30 minutes before their scheduled time.
  • Assist patients with telehealth setup, ensuring the functionality of video and audio systems.

Patient and Provider Support:

  • Notify providers of patient readiness and relay key visit details.
  • Troubleshoot technical issues for patients before and during visits.
  • Verify completion of all required consent forms and pre-visit documents.
  • Address last-minute changes, including cancellations or no-shows, and optimize provider schedules using waitlists.

Requirements:

Education:

High School Diploma or GED required.

Associate or Bachelor's degree in Healthcare Administration, Public Health, or a related field preferred.

Experience:

1-2 years of experience in healthcare administration, patient admissions, or care coordination.

Proficiency with systems such as Salesforce, AdvancedMD, or ChronicareIQ is highly desirable.

Skills:

Exceptional attention to detail and organizational skills.

Outstanding communication and customer service abilities.

Proven ability to multitask and manage competing priorities in a fast-paced environment.

Strong problem-solving skills with a focus on patient-centered outcomes.

Technical Proficiency:

  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Familiarity with healthcare CRM and EMR systems, including Salesforce, AdvancedMD, and ChronicareIQ.
  • Experience with telehealth platforms such as Doxy.me or Zoom.
