Overview

Responsibilities:

  • Conduct comprehensive assessments of patients with diabetic disorders.
  • Develop and implement individualized treatment plans.
  • Monitor and adjust treatments based on patient progress and lab results.
  • Educate patients and their families about endocrine disorders, treatment options, and lifestyle modifications.
  • Collaborate with endocrinologists, primary care physicians, and other healthcare professionals.
  • Perform diagnostic tests and interpret results.
  • Prescribe medications and manage medication regimens.
  • Provide follow-up care and manage chronic endocrine conditions.
  • Participate in clinical research and quality improvement initiatives.
  • Maintain accurate and up-to-date patient records.

Qualifications

REQUIRED QUALIFICATIONS

  • Master's degree or higher in nursing or physician assistant studies from an accredited program
  • Current Illinois license as a nurse practitioner or physician assistant
  • Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
  • Minimum of two years of experience as an APP
  • Passion for Diabetic Managment
  • Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
  • Proficiency in electronic medical record (EMR) systems
  • Expectation within 3 years obtain the Advanced Diabetes Management Board Certification
