Advanced Care Practitioner - Comprehensive Sickle Cell Clinic The Comprehensive Sickle Cell Clinic is seeking an Advanced Care Practitioner (Nurse Practitioner or Physician Associate) to provide specialized care for patients with sickle cell disease. This role involves delivering high-quality, patient-centered care across various settings, including inpatient care, infusion center and routine health maintenance visits. The practitioner will work closely with a multidisciplinary team to address the complex medical needs of sickle cell patients, ensuring comprehensive and compassionate care. Key Responsibilities : Clinical Care : Perform thorough assessments, including history taking, physical examinations and blood work, for patients in crisis and routine visits. Develop and implement individualized treatment plans for sickle cell patients, including prescribing full and partial opioids and managing refills. Provide chronic pain management, including pain contracts when necessary and narcotic tracking. Monitor and adjust disease-modifying therapies, such as hydroxyurea, based on patient response and diagnostic results. Manage chronic transfusion patients and coordinate with apheresis when necessary. Coordinate inpatient care to ensure individualized care plans are followed and patients are promptly discharged back into care. Educate patients and families about sickle cell disease, treatment options, and expected outcomes. Document patient care, dictating clinic notes and communicating with referring physicians. Participate in clinical trials, including patient enrollment, health assessments, and data management. Attend weekly sickle cell team meetings. Present at national sickle cell meetings as time allows. Multidisciplinary Collaboration : Participate in the SAPPORT (Sickle Cell Disease Advanced Practice Providers Opportunities Resources and Training) Program. Attend monthly SAPPORT ECHO sessions. Work within our team of nurses, social worker, researchers and SCD experts to provide holistic care. Attend and contribute to clinic meetings and program leadership discussions. Act as a liaison between the clinic and other healthcare providers to ensure continuity of care including inpatient care. Patient and Family Support : Provide compassionate support and counseling to patients and their families. Address and resolve patient concerns and complaints professionally and empathetically. Foster long-term relationships with patients, promoting trust and adherence to treatment plans. Professional Development and Education : Stay current with advancements in sickle cell disease treatment and care. Provide education and training to other healthcare professionals on sickle cell disease management. Participate in continuing education and professional development opportunities. Qualifications : Master's degree in Nursing or Physician Assistant. NP or PA licensure in Virginia. Current CPR certification from the American Heart Association or American Red Cross. Minimum of three years of clinical experience, preferably in hematology/oncology or chronic disease management [AS1]. Strong oral and written communication skills. Ability to work independently and as part of a multidisciplinary team. Excellent customer service skills and the ability to work in a fast-paced environment.
