Med-Scribe is proud to present an opportunity for a Nurse Practitioner or Physician Assistant on behalf of an esteemed healthcare center dedicated to serving the underserved in Rochester, New York. With over 30 years of impactful service, this organization provides essential primary medical, dental, and mental health care to individuals experiencing homelessness.

As an NP or PA, you will offer comprehensive primary care primarily in a clinic, with the option to provide care within community centers, shelters, and on a mobile medical unit. Key Responsibilities:

• Provide comprehensive primary care primarily in a clinic setting, with the option to assist in shelters and on a mobile outreach van.
• Perform minor outpatient procedures and consult with other healthcare professionals as needed, ensuring evidence-based practices are followed.
• Assess patient needs, make appropriate referrals for specialty care and diagnostic services.
• Educate patients on health maintenance, disease prevention, and treatment, focusing on harm reduction techniques.
• Complete timely documentation in patient records, maintain necessary credentials, and participate in quality improvement initiatives.

Salary and Benefits:

• Competitive salary of $100,000 to $115,000 per year.
• Comprehensive benefits package.

Schedule:

• Monday through Friday, 8:00 AM to 4:30 PM.
• Optional evening shifts available from 6:00 PM to 9:00 PM.

Qualifications and Requirements:

• Minimum of two years' experience as a licensed Nurse Practitioner (FNP preferred; ANP will also be considered) or Physician Assistant.
• Experience in a family practice clinic setting preferred.
