Post Discharge Advanced Practice Provider

9a-5p Mon-Friday

Hybrid/Telehealth with future on-site coverage required in Stamford/White Plains

Overview:

An Advanced Practice Provider working with designated HSS physician(s) providing for and facilitating outcomes of care for the defined patient population. Evaluates and synthesizes clinical findings that result in optimal goal achievement throughout the continuum of care. Integrates theory, research, and critical thinking in the delivery of care to patients/families and serves as a resource in complex patient and family situations. Position may include the diagnosis of illness and physical conditions and the performance of therapeutic and corrective measures within a specialty area of practice, in collaboration with a licensed physician qualified to collaborate in the specialty involved, provided such services are performed in accordance with a written practice agreement and written practice protocols. Prescriptions for drugs, devices, and immunizing agents may be issued in accordance with the practice agreement and practice protocols.

Roles and Responsibilities:

  • Adheres to and promotes the HSS mission and values, HSS Standards of Care and Code of Conduct practices.
  • Incorporates patient rights into the plan of care and in conjunction with the health care team.
  • Communicates clearly, explains all procedures, treatments, and medications to patient and family and answers questions within the scope of clinical practice.
  • Completes annual Joint Commission, State and Hospital mandated inservices.
  • Participates in the development and upgrading of interdisciplinary and nursing standards, and patient/family education materials and programs as relevant to expertise and practice.
  • Act as a resource by providing consultation and educational programs to nursing and medical staff as requested and indicated.
  • Assists in developing and monitoring patient outcomes per clinical indicators established within practice pattern for purposes of QA/PI.
  • Initiates and enhances professional growth and development by keeping current with changes in the healthcare environment, legislative changes, and with participation in Hospital, local, national and international educational meetings.
  • Publishes/disseminates new practice standards through scientific and professional publications and presentations.
  • Identifies patterns and trends in the needs of patients and caregivers within the specialty that may indicate a need for further analysis and possible change in practice.
  • Maintains satisfactory attendance record.
  • Manages time appropriately to meet patient, department and Hospital needs.
  • Performs physical examination, including disease-specific exams (eg neuro, etc) and correctly identifies abnormalities.
  • Obtains and reviews patient record and medical history, extracting relevant data.
  • Orders appropriate diagnostic tests, reviews results and identifies findings requiring intervention or follow-up.
  • Collaborates with physicians, peers and other members of the health care team to facilitate and coordinate care to an established patient population from initial assessment to discharge.
  • Collaborate with interdisciplinary team to assess, develop, implement, and coordinate plan of care based on findings and patient needs.
Qualifications:

Requirements:

NP Requirement:

MSN from Accredited School of Nursing

Obtains/Maintains NP licenses in New York and CT

FNP Required

Obtains/Maintains board certification as a NP in specialty area of practice

BLS/ACLS via American Heart Association

DEA

PA Requirement:

BS or MS in Physician Studies from an accredited program

NCCPA certification

Obtains/Maintains PA licenses in New York and CT

BLS/ACLS via AHA

Infection Control Certificate

Child Abuse Mandate

Both: Three Years Experience and use of Epic EMR and Orthopedic based practice preferred

Other Requirements: #LI-Onsite Pay Range - Minimum: USD $140,662.00/Yr. Pay Range - Maximum: USD $161,662.00/Yr.
