From their love and commitment Baywell Health started out as The West Oakland Health Council. We quickly became the neighborhood's anchor for healthcare, health education, food and housing programs, and voter registration. Fifty years later, we continue to be the Bay's home for top-notch healthcare, health education, and other services intended to care for the whole you, and the whole family. Legacy of Love Video: Healthcare Rooted in West Oakland Job Summary: Under the general supervision of the Community Health Program Manager, the Community Health Advocate III works to schedule and coordinate all Community Health Program community and patient outreach initiatives. They will serve as a community liaison who will establish and maintain relationships with local health, service, and program partner providers. Duties/Responsibilities: Create and manage Baywell Health's patient and community outreach calendars. Schedule Healthy Community Department participation in or hosting of community events. Manage and finalize home visit and clinic appointment scheduling for out-of-care patients. Lead an integrated care team working with CHA's, clinical staff, and support teams to ensure a responsive patient healthcare experience. Prepare and communicate across care functions to ensure patients are equipped for clinical appointments. Conduct screenings to identify unmet social determinants of health (e.g., homelessness, food insecurity, transportation needs). Document and review findings to identify patients requiring higher levels of care management. Assist with Medi-Cal applications at community events. Coordinate health services, referrals, and ongoing care as part of an integrated health team. Act as a personal guide and advocate for patients, ensuring culturally concordant care. Engage community members to encourage participation in health programs. Coordinate case management services for patients with complex needs. Attend meetings with county agencies to coordinate services for low-income families. Foster partnerships with local organizations, businesses, and agencies to promote community health and engage patients. Collect, analyze, and enter health data to assess program effectiveness and identify areas for improvement. Promote healthy lifestyles and behaviors through outreach and education. Participate in MyChart outreach, chronic condition education, new member engagement, and retention efforts. Plan and execute health programs addressing community concerns. Participate in Plan-Do-Study-Act (PDSA) initiatives to improve access, care, and health outcomes. Attend trainings and care coordination meetings. Provide backup for other CHA's or team members. Performs other related duties as assigned. May be assigned cross departmental responsibilities. Required Skills/Abilities: Ability to promote health equity through programs and policies. Exhibit cultural humility and bridge communication gaps between patients and providers. Proficiency in Microsoft Office Suite (Excel, Word, etc.). Preferred experience with Next-Gen EHR, Epic, and Tableau. Ability to manage health data and reporting tools. Ability to project a positive and concerned public image. Understanding of clinical healthcare, public health practices, and health equity. Familiarity with healthy lifestyle, nutrition, and fitness practices. Ability to remain calm and courteous in high-stress environments. Work independently with minimal direction. Reliable transportation and willingness to travel between clinics. Excellent verbal and written communication skills. Excellent interpersonal and customer service skills. Excellent organizational skills and attention to detail. Excellent time management skills with a proven ability to meet deadlines. Ability to function well in a high-paced and at times stressful environment. Must possess a valid driver's license. Education and Experience: Bachelor's degree in Health, Healthcare, Social Welfare, Psychology, Public Health, or a closely related field preferred. Equivalent combinations of education and experience considered. All CHA's will participate in training to become certified Community Health Workers (CHWs). All CHA's will participate in training to become Certified Enrollment Counselors. CHA's will be expected to seek and complete other health professional development training courses related to family planning, lay counseling, food handling, and other topic areas as appropriate. 1-2 years of experience in a community health setting. Planning and conducting community outreach programs. Experience as a lead care manager coordinating physical, mental, and dental care. Working with underserved and minority populations preferred. Specific vision abilities required by this job include close vision, peripheral vision, and the ability to adjust focus. May be required to work in an environment with high noise levels and unpredictable temperature and ventilation.
