Program Purpose:

The African American Elders Program serves African-American elders in Central and Southeast Seattle and South King County. The program identifies frail, isolated and hard-to-serve African Americans and assists them in accessing and receiving needed social and health services through ongoing case management, support, referrals, and prevent premature institutionalization.

Position Description:

The African American Elders Program Director is responsible for actualizing the mission of the African-American Elders Program. The mission is to improve the quality of life of isolated, frail, and low-income African-American elders in King County promoting accessible, high quality, culturally appropriate and comprehensive services. The AAEP identifies isolated African-American elders through outreach and assists them with access to social and health services through ongoing case management, support and referrals. The Program Director must track effectiveness of services through statistical analysis, consumer feedback, and community surveys. This position oversees a program assistant, care coordinators, counselors and nurse(s). This position is responsible for a budget of $1.9 million.

Major Duties and Responsibilities

  • Creates and nurtures effective partnerships with community resources that benefit frail African-Americans or their families
    • Identifies community resources such as churches, community centers, programs specifically aimed at African-Americans, professional groups, clubs, and medical resources.
    • Establishes working relationships with the resources that may include but not be limited to offering training, information, and access.
    • Offers training to key staff in the community resources in how to identify frail elders in need and how to access services.
    • Provides informational activities that may include workshops, fairs, flyers, etc. to the community resources either as a whole or individually.
    • Works in conjunction with other programs within the aging network.
  • Creates effective partnerships with programs that offer social, medical, or mental health services to frail elders, to ensure culturally relevant services are provided
    • Identifies agencies that offer services to frail elders.
    • Establishes working relationships with the agencies to include a standard Memorandum Of Understanding.
    • Offers training on how to be a culturally relevant provider.
    • Coordinate with community partners and/or contractors; including participation in applicable community meetings; ensuring all relevant information is disseminated timely, appropriately and accurately.
  • Coordinates activities of the AAEP
    • Responsible for assuring all AAEP staff are aware of all aspects of the AAEP.
    • Coordinates activities with all AAEP staff – for example, before making a presentation to a community group or stakeholders, makes sure Care Coordinators are aware so they can expect potential new cases.
    • Solicits and puts to use feedback from all AAEP staff to improve the way services are offered.
    • Coordinates program operations in consultations with the City of Seattle and CCS.
    • Plans, evaluates and implements program and staff development and training.
    • Reviews and approves assessments and care plans developed by Care Coordinators.
  • Grant/Fundraising
    • Assists CCS fundraisers in raising funds for the AAEP. This may include assisting with writing an appeal letter, identifying potential donors, etc.
    • Grant renewal collaboration with CCS in researching and writing grants to assist frail African-American elders, expanding the AAEP, and offering new services.
    • Monitor overall program budgets to include personnel, non-personnel, operational and administrative expenses.
    • Coordinate with Accounting for compliance of grant expenditures and allowable costs to include communicating budget changes and expense tracking.
    • Ensure Contract Compliance & Reporting.
    • Be familiar with grant policies and procedures as well as allowable expenses, budgets and coordination of services.
    • Ensure timely and accurate completion and submission of required reports to funders and other applicable entities.
    • Prepare for and manage program monitoring's, including any responses to concerns or findings.
  • Program Evaluation: Tracks the effectiveness of the AAEP through statistical analysis, consumer feedback, and community surveys.
    • Statistical analysis: establishes baseline of usage of services by African-American elders, compares trend at appropriate annual or other intervals; assures client contacts, client service information and other relevant indicators are tracked (can be tracked by Program Assistant); analyzes and prepares reports on statistics and indicators.
    • Participate in the federal database to coordinate activities across the US.
    • Consumer feedback: creates, implements, analyzes, and prepares reports on consumer survey. Consumers include the frail elders as well as community resources who work with the AAEP.
    • Community survey: creates, implements, and analyzes community survey – usually an annual activity.
    • Review, analyze and interpret current legislative and regulatory changes in program mandates.
    • Prepare monthly reporting for state and local funders.
    • Suggests improvements to aspects of the AAEP based on analysis from data, indicators, or feedback.
    • Ensures compliance with Washington State Standards for Care Coordinators.
  • Access to and protection of PHI/ePHI:
    • Holds (primary) leadership role in creating a culture of privacy and security of Protected Health Information in HIPAA Covered Entity service area, including education of staff about their responsibilities for safeguarding privacy and security.
    • Authorizes level of staff access to PHI/ePHI to ensure staff can perform assigned job duties; regularly monitors access to ensure appropriate safeguarding of PHI/ePHI.
  • YEARS OF EXPERIENCE/EDUCATION

    • Bachelor's Degree in Human Services, Social Work, Business Administration, Political Science or a closely related field with a minimum of five years of management experience.
    • Minimum 3 years' experience in the field of geriatrics.
    • Preferred: Master's Degree in Social Work or Nursing and experience working in a social services program that services African-Americans.

    • Success in a governmental or non-profit organization with a proven track record of achieving results in program development.
    • Understand the principles of social service programs and administration.
    • Advanced knowledge of public health and geriatric issues of older adults.
    • Federal, state, and local laws, rules and regulations governing client care.
    • Case worker objectives, principles and methods.
    • Principles and practices of employee supervising and staff development.
    • Working skills in Microsoft Suite, Adobe, and database management/reporting systems.
    • Excellent communication and interpersonal skills.
    • Strong organizational and time management skills with the ability to work within systems.
    • Ability to work as part of a team or independently.
    • Previous work history in senior services.
    • Demonstrated ability to work with a diverse workforce, to include knowledge of culturally relevant services to African-Americans.
    • Car with valid Driver's License and automobile insurance.
