DescriptionResponsibilities & DutiesThe Aftercare Coach is responsible for working with the Program Department to provide advocacy and facilitate deliverable [program] services. The Aftercare Coach is directly involved with providing quality services to our guests in an environment of love, grace, dignity, and respect. Transformation is a plan to support our guests in the final stage of their program and continue to support them after their program is complete. Oversee classes, and groups, help with relapse prevention tools, and help them stay in community as they work toward their life goals while practicing the tools learned in the program.
  • Kindly greet all guests, including, but not limited to, visitors, volunteers, vendors, staff, guests, donors; community groups, church groups, and public officials
  • Establish and build healthy Christ-honoring relationships with guests, while maintaining the Code of Ethics as it pertains to professional certification [i.e. Pastoral, Coaching, Counseling]. The Aftercare Coach does not focus on clinical work, but they must still be capable of responding in professional, ethical ways, including cultural responsiveness and trauma-informed best practices
  • Under the leadership of the Recovery Services Manager, maintain the Standards of Excellence and actively participate in the process of upholding this standard
  • Comply with the HIPAA Compliance, Privacy, and Security Rules and GRM's Confidentiality Policy.
  • Maintain thorough and timely guest records in a web-based tracking system, document any interactions, concerns, or other pertinent information regarding guests, etc.
  • Transformation includes activities, classes, interventions, and resources to help a person in recovery utilize tools to cope with triggers, stress, and cravings that they may face when program is over including reaching out to their support team, spiritual mentor, sponsor, therapist, church, pastor, etc.
  • Assess guest needs for physical, medical, and behavioral intervention, providing resources and guidance as appropriate
  • Maintain a varying caseload in the Transformation Phase, meeting regularly with the guests.
  • Ensures that guests are engaged with necessary clinical and/or services in the community, helps transition happen smoothly, and ensures that the work of multiple partners is coordinated.
  • Work with guests to finalize long-term recovery Vision Plans.
  • Use evidence-based or best practices.
  • Coordinate with the Workforce Development and Shelter Advocates team to track guest progress.
  • Collaborate with the Job Training Center to facilitate internal referrals and track guest progress.
  • Verify guest employment status and document as needed.
  • Monitor guest financial status and document savings and expenditures.
  • Check and respond promptly to voice mail and email.
  • Provide support/coverage in the event of staff absence as directed.
  • Flexible hours to meet guest work schedules.
  • Aftercare Services (Alumni)
    • Meet Aftercare goals (i.e., housing, employment, sobriety, church involvement) by following up with guests who have completed the Recovery Program
    • Organize and oversee meetings, outings, and quarterly check-ins.
    • Oversee Out-Patient Program
  • Follow up with the guest recovery support team to track participation in offsite recovery activities.
  • Attend professional training and networking opportunities as directed
  • Attend staff meetings or other meetings as directed
  • Perform other duties as assigned
Employment Status:
  • Full-time 40 hours per week nonexempt, eligible for paid time off; eligible for benefits
  • Anticipate flexible schedules
Reports to: Recovery Services ManagerVacation level: 2Requirements
  • Minimum high school diploma or GED.
  • Coach Certification, Pastoral Studies, or Addiction Counseling Degree
  • 5+ years of operational experience in the capacity of a coach, leading and counseling as a pastor, or licensed addiction counselor
  • Substance use disorder recovery experience, dealing with addictive behaviors, (including but not limited to substance use disorder, chemical dependency, eating disorders) trauma, depression, and mental health
  • Proficient in Microsoft Office and web-based management systems, proficient listener and problem solver, creates an environment of social positivity, and a passion for serving people
  • Maintain a valid Arizona Driver's License and a clean driving record
  • Bilingual - English/Spanish a plus
  • Must be an evangelical Christian, living a life dedicated to Christ, and be an instrument of God's love, grace, hope, and forgiveness to all
Physical Demands:
  • May encounter individuals who are ill and/or contagious and must take precautions to ensure the health and safety of everyone
  • Requires frequent sitting, standing, and walking; and lifting, carrying, pushing, and/or pulling objects weighing up to 50 pounds

  • The population we serve is typically in crisis. This can cause emotional stress and compassion fatigue. GRM highly promotes employee self-care.
