Provides counseling, advocacy and chemical dependency treatment services to patients.
Job Responsibility
1.Interviews new patients and provides program orientation. 2.Conducts psychosocial assessment related to chemical/substance abuse and dependence. 3.Collaborates with healthcare team members to develop and implement patient treatment plan. 4.Conducts individual, group and family counseling sessions. 5.Provides transitional care services to facilitate patient discharge. 6.Completes documentation in accordance with policies and procedures of the organization, State and Federal regulations. 7.Serves as a liaison to referral sources to facilitate continuity of care. 8.Participates in related utilization review and performance improvement activities. 9.Performs related duties as required.
Job Qualification
*High School Diploma or equivalent, required.
