Job Type Full-timeDescriptionProvides adequate contact with clients in the apartment program to ensure the health and safety of residents and to ensure appropriate support is given for other community services. Observes and documents resident's behaviors to facilitate assessment of treatment goals. This position may require use of personal vehicle to transport clients when an agency vehicle is not available.ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Immediately reports maintenance concerns/issues to the Apartment Program Director.
  • Assists in the provision for the physical needs of each resident by the following means: assist and monitor each resident with obtaining adequate food, clothing and shelter, monitors the self-administration of medication by residents and the adherence to physician's instructions.
  • Assist/Instruct residents with how to properly store medications and valuables.
  • Reports to the supervisor all pertinent information concerning illnesses, accidents, untoward events, staff difficulties and repair needs.
  • Monitors food, supplies and upkeep in the apartment helping the resident make appropriate purchases or assisting in the cleaning whenever necessary.
  • Develops and monitors service plans with individual residents using principles of Psychiatric Rehabilitation.
  • Assists in the delivery of restorative services as designated on the service plan.
  • Meet consistently with each resident to provide one-to-one support.
  • Provides transportation for residents to other needed services with the assistance of other staff.
  • Organizes therapeutic activities which include socialization (discussion groups) and recreational opportunities (sports, games, arts and crafts, music).
  • Assist with resident relocation including moving of personal items and cleaning of previous rental site.
  • Resource Management: Obtains a thorough knowledge of appropriate community resources, serves as an advocate for residents in securing needed services from other agencies, refers residents to appropriate social, rehabilitative, medical, mental health and vocational services.
  • Case Documentation: Evaluates each resident's growth potential by utilizing the needs assessment tool and the consumer's self-assessment, Gains input from the team in order to develop service plan goals, documents case records and progress notes, obtains ongoing required medical records, assist with completing chart audits as assigned.
  • Working as a Member of a Team: Establish and maintain a positive working relationship with co-workers emphasizing the following: Doing your share of work, leave work area clean and/or neat, shows a level of tolerance and flexibility towards others, Gives and/or receives positive and constructive feedback, carries our team decisions consistently.
Management has the right to add or change this position at any time.Work Schedule: Monday- Friday 8:00 am- 4:30 p.m. and one rotating Saturday every 5 to 6 weeksRequirementsEDUCATION and/or EXPERIENCE:Required: Associate's degree in the Human Services field or equivalent from two-year College or technical school; and two years related experience and/or training; or equivalent combination of education and experience.Preferred: Bachelor's degree in the Human Services field; and two years related experience working and/or training in the mental health field (including working with clients who are developmentally delayed, geriatric or have co-occurring disorders), and QMHS.Salary Description $17.50-$18.30 per hour
