The practice is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion .We are currently looking for a APCM Coordinator to join our team. Responsible for collecting and analyzing data, preparing reports, communicating with clinicians and patients, and managing the operations of the Advanced Primary Care Management program.Responsibilities1. Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers2. Updates chronic care management data in patient medical records under a clinician's directions3. Runs reports to facilitate chronic care management reporting and billing4. Provides administrative support to the department as needed5. Adheres to the organization's policy in regards to absenteeism and appearanceQualifications1. College degree preferred 2. Must be a Certified Medical Assistant from an approved school 3. 6+ months of program coordination and medical assistant experience4. Experience with NextGen electronic medical record a plus5. Knowledge of and adherence to HIPAA6. Strong computer skills 7. Excellent customer service and interpersonal skills 8. Strong organizational skills9.
