Job Title: Assembly and Analysis Clerk Reports To: MANAGER - HIM Direct Reports: None FLSA Classification: Non-ExemptPosition Summary The Assembly Clerk is responsible for providing outstanding customer service while performing a variety of clerical duties involved in maintaining patient medical records to include assembly, analysis, filing, retrieving and distributing medical records, loose notes and reports and maintaining associated logs and records in accordance with established priorities and procedures within the Medical Record Area. Processes Release of Information requests in accordance with current HIPAA regulations. General Duties, Tasks and Responsibilities
- Analyzes patient records for deficiencies in an accurate manner; documents or removes deficiencies utilizing appropriate manual and computerized systems; assembles, scans and indexes patient records in an accurate and timely manner; performs validation of scanned records.
- Answers telephone and takes requests for medical records or information; provides routine information of a general nature such as history numbers, admission or discharge dates and medical record locations to authorized requestors.
- Copies medical information from patient medical records to answer authorized requests in support of patient care activities, patient account settlements, and third party sponsorship; documents patient records appropriately.
- Signs out charts from the department utilizing appropriate manual and computerized systems.
- Sorts or files medical records, outpatient reports and loose reports in a timely manner; sorts requests according to predetermined priorities.
- Follows department Release of Information policies and procedures.
- Participates in performance improvement activities.
- Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
- Performs other duties as assigned.
Education Requirements
- High school diploma/or equivalent
Experience Requirements
- Office / Clerical setting minimum 1 year Preferred
- Healthcare Insurance / Billing minimum 1 year Preferred
Computer Skills
- Strong computer skills including Microsoft Word, Excel and Outlook
