Assessment Specialist - GA: Laurens and Surrounding CountiesReady to join a growing company whose work profoundly impacts people's lives in their community? CareStar, Inc. is currently seeking an Assessment Specialist in the Clinical department to identify, coordinate, and oversee the delivery of services to enhance the health, safety, and general well-being of participants.THE POSITIONCareStar is looking for motivated individuals who: Possess a Bachelor's degree in social work, sociology, psychology, human services, or a related field, orAre a registered professional nurse currently licensed to practice in the State of Georgia AND have two years' experience in the human services or health-related field, orPossess an associate degree in human services or health-related field AND have a minimum of two years' experience in working directly with aging or disability populations.Have specialized knowledge of older persons, the aging process, long-term care issues, and persons with disabilities, and know disability etiquette.Demonstrate effective organizational and communication skills.Exhibit willingness and ability to work a flexible schedule to meet the needs of families, including reliable transportation, valid driver's license, and car insurance as required by state law, and the ability to travel.Adhere to the CareStar Rule in the performance of job responsibilities.Understand and comply with CareStar Policies and Procedures.Maintain confidentiality as related to patient information. Any unlawful disclosures of confidential information will be treated as a serious disciplinary offense.Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
