Job Description :
The shift available at this location (New Directions and Garden City) is: 40 hours per week (Mon-Fri 7:30am to 3:30pm - some flexibility needed). The salary rate for this position is $62,400 per year..Life's WORC offers a great benefits package, including:

QUALIFICATIONS
  • Bachelor's Degree in Human Services preferred
  • Minimum four years' ID/DD or related human services background
  • Minimum of one year's supervisory experience required
  • Experience with OPWDD HCBS waiver regulations
  • Excellent verbal and written communication skills
  • Valid Driver's License required
  • Proficiency in computer skills required
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO
  • Oversee and supervise the day-to-day operations of assigned Day Habilitation sites as it pertains to individuals and staff to include monthly site-specific staff meetings, community support checks and implementation of emergency protocols
  • Direct supervision to the Day Habilitation site Supervisors to include consistent support, coaching and training, monthly Supervisor/Cluster Meetings and 1:1 supervisory support meetings.
  • Provides assistance to all Day Habilitation programs as needed.
  • Interview, hire and train Supervisory staff and direct support professional as needed.
  • Oversight of program personnel related issues/systems to include payroll, PTO, performance evaluation, progressive discipline, training, recruitment, and retention.
  • Collaborate with other support departments such as Community Habilitation, Customized Employment and Respite services.
  • Ensure program admission coordination is being followed by Day Services Supervisors
  • Ensure regulatory compliance and oversight as per OPWDD regulations and Agency P&P to include Medisked, Fleetmatics, incident management, fire safety/evacuation plans & drills, SAP checklist monitoring and maintenance.
  • Sample review Medisked daily and monthly notes
  • Assist in crisis intervention and provide support and guidance.
  • Participate in and/or facilitate meetings, in-services/training or committees as needed or assigned.
  • Ensure adequate provision of ABA related services are being monitored by the day services supervisors.
  • Ensure Staff Action Plans are consistent with person centered principles via regular review and approval process.
  • Ensure adequate staffing coverage at all times.
  • Collaborate with DH Director to ensure Day Habilitation sites remain within budget to include monitoring of Medisked documentation/billing and unit utilization.
  • Collaborate with other Assistant Directors to ensure systems standardization across all program sites
  • Participate and cooperate when an incident of any kind is filed internally or through the Justice Center and an investigation is initiated.Includes participating in interviews with an investigator as requested and providing a signed & dated written statement to the Q.A. Investigator within 24hrs of completing the interview.
  • Completion of Assistant Director reports
  • Perform other duties as assigned
