Title: Assisted Outpatient Treatment (AOT) Supervisor- Position located in Middletown-in personReports to: Assistant Director of Care Coordination ServicesJob Objectives:Under the direction of the Assistant Director of Care Coordination Services, assist with the oversight of Mental Health Association in Orange County Inc.'s (MHA) Assisted Outpatient Treatment (AOT) program and Provide Health Home Plus Care Management to individuals participating in the Assisted Outpatient Treatment (AOT) program who are living with serious mental health disorders as they transition back into the community from the NYS Correctional System, NYS Psychiatric Hospital System or other settings.Maintain a caseload of 8-10 Assisted Outpatient Treatment (AOT) Health Home Plus ParticipantsPerform duties while actively promoting an environment consistent with MHA's Cultural Equity Policy and WELCOME Orange.Responsibilities:
  • Attend bi-weekly Care Coordination team meetings.
  • Responsible for maintaining a caseload of up to 10 Assisted Outpatient Treatment (AOT) Health Homes Plus program participants.
  • Responsible for completing required data collection, statistical, and program reports, and other administrative reports.
  • Responsible for the direct supervision of AOT care management staff.
  • Will provide leadership and support by facilitating and leading multidisciplinary teams of medical, mental health, chemical dependency treatment providers, courts/legal entities, social workers, nurses, representatives of the Local Government Unit (LGU) as directed by the county AOT Coordinator and other care providers toward assuring enrollees receive services as indicated in the enrollee's single plan of care.
  • Conduct four monthly in person visits with clients in the community
  • Provide 24-hour on-call services when scheduled.
  • Attend monthly Orange County AOT/Forensic Care Management meetings
  • Maintain all client records electronically through Foothold Technology and CAIRS Electronic Record Keeping.
  • Represent the agency at the weekly Single Point of Access (SPOA) meetings, as needed.
  • Attend trainings and meetings relevant to Assisted Outpatient Treatment (AOT).
  • Review and ensure all Assisted Outpatient Treatment (AOT) program participants' files adhere to compliance guidelines (i.e. Medicaid, Office of Mental Health and Department of Health).
  • Complete monthly billing for assigned caseloads.
  • Establish and maintain linkage with appropriate community agencies and service providers.
  • Represent MHA at meetings and events as necessary.
Other Duties:
  • Participate in MHA's committees as needed.

RequirementsEducation and experience:Education
  • A Master's degree in one of the qualifying fields; and two (2) year of Experience; OR
  • A Bachelor's degree in one of the qualifying fields; and three (3) years of Experience; OR
  • A Bachelor's degree or higher in ANY field with either: five (5) years of Experience, or two (2) years of experience as a Health Home care manager serving the SMI or SED population.
  • Qualifying fields: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing;
Experience must consist of:
  • Providing direct services to people with Serious Mental Illness, developmental disabilities, alcoholism or substance abuse, and/or children with SED; OR
  • Linking individuals with Serious Mental Illness, children with SED, developmental disabilities, and/or alcoholism or substance abuse to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing and financial services).
  • Experience linking people who have been diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse to a broad range of services essential to successfully living in a community setting.
  • Effective written and oral communication skills, including public speaking ability and experience.
  • Proficiency in Excel and Microsoft Word required.
  • Experience with Health Homes highly preferred.
  • Knowledge of Medicaid procedures highly preferred.
  • Experience with Electronic Health Record Program(s) required.
  • Ability to work independently with minimal direct supervision.
  • Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.
  • Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
  • Valid Driver's License and ability to drive with current driving record acceptable for agency insurance coverage.
