Licensed Practical Nurse Job DescriptionProgram •Albuquerque, New MexicoDepartment: ProgramEmployment Type: Full-TimeMinimum Experience: Licensed NurseCompensation: Up to $34.00 an hour, Depending on Experience. BASIC RESPONSIBILITIES AND DUTIES:
  • Provides quality and safe delivery of health care services under the direction of the registered nurse.
  • These duties may vary but include: All required nursing activities under the Nurse Practice Act, and regulations and standards, required nursing activities for the Developmental Disabilities Medicaid Waiver program and the ICF/IID programs the company participates.
  • Appropriate health teaching for direct support personnel, person served, family members and guardians relating to medical condition(s), medications, self-care, and answering question.
  • Skilled observations, recording, and timely reporting to the physician, RN, and/or Nursing Supervisor any pertinent changes in the person served condition or needs.
  • Assisting with health care according to the Individual Service Plan (ISP) , or Individual Program Plan (IPP).
  • Documents the care and pertinent person served reactions at each visit and completes nursing progress notes according to rules, regulations, in Therap and company policy.
  • Enters all health care information into THERAP.
  • Assumes responsibility for personal growth by attending staff meetings, and in-service training by the RN or company trainer.
  • Provides cooperative effort toward quality care by understanding functions of other members of the team.
  • Accepts the philosophy, purpose and policies of the company.
  • Provides on-call telephone triage as scheduled to assess and maintain the physical health and mental health of persons served.
  • Maintains a cooperative relationship among health care teams , interdisciplinary team by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Interpret, and evaluate diagnostic tests to identify and assess person's served condition
  • Attends physician visits or other health care provider visits to ensure that proper information is relayed to provider and care is provided at the individual's home or habilitation program or work.
  • Observe less skilled nursing and direct support personnel and visit persons served to ensure that proper nursing care is provided.
  • Provides required Quality Improvement activities relating to nursing and pharmacy and medication administration and health care services
  • Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
