The Behavioral Health Care Manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.In this role you will be responsible for:Supporting the mental and physical health care of patientsScreening and assessing patients for common mental health and substance abuse disordersFacilitating patient engagement and follow-up careProviding patient education about common mental health and substance abuse disorders and the available treatment optionsSystematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complicationsSupporting psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatmentProviding brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriateProviding or facilitating evidence-based psychosocial treatmentsParticipating in regularly scheduled caseload consultation with the psychiatric consultant and communicating resulting treatment recommendations to the patient's medical providerTracking patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.Documenting patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, psychiatric consultant, and other treating providersFacilitating treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultantFacilitating referrals for clinically indicated services outside of the organizationDeveloping and completing relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be dischargedRequired QualificationsBachelor's degree from an accredited school in Social Work, Nursing, or PsychologyMinimum 2 years of social work and/or care management experienceCurrent and valid driver's licenseReliable transportation and valid automobile insurance coverageDemonstrates knowledge of differential diagnosis of common mental health and/or substance abuse disordersExperience with screening for common mental health and/or substance abuse disordersExperience with assessment and treatment planning for common mental health and/or substance use disordersExperience with evidence-based counseling techniquesAbility to effectively engage patients in a therapeutic relationshipDemonstrates knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disordersDemonstrates basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling roleDesired QualificationsMinimum 5 years of social work and/or care management experienceFamiliarity with Aprima Electronic Medical Record (EMR) systemLocationOffice Location: 770 Kenmoor Ave, Grand Rapids, MI 49546Main Service Area: Grand Rapids and Surrounding AreasHoursOffice Hours: 8:00 am - 5:00 pm, Monday through FridayBackground ScreeningCareline Health Group conducts a background screening upon acceptance of a contingent job offer.
