This role involves assessing clients' needs, assisting them in accessing treatments, and connecting them with appropriate resources and social services. The Case Manager advocates for clients, ensuring they receive comprehensive care to improve their health and well-being. The Behavioral Health Case Manager will assist with outreach to Promise Healthcare patients identified with behavioral health needs and further screening or assessment. The BH Case Manager will work collaboratively with the health navigator and Behavioral Wellness staff.Essential Duties and Responsibilities:
- Assist in continuously developing and enhancing a process for external patient referrals from partner organizations.
- Assist in the continued enhancement of a process for internal referrals for Promise Healthcare patients identified through screenings as needing further assessment or services.
- Conduct outreach to patients and community members identified as needing services and assist them in accessing them. This includes helping with appointment scheduling.
- Work with Promise Clinical staff and enable staff, including Community Health Workers, Case Managers, and Health Coordinators, to address identified Social Determinants of Health, including transportation, food insecurity, and other social determinants.
- Work with Promise Referral Coordinators to help patients access health care services.
- Work with Promise Enrollment staff to help patients and community members enroll in Medicaid, Marketplace products, and Promise Sliding Fee Scale assistance.
- Assist with garnering patient feedback and working collaboratively to address barriers to patients' care and vaccinations.
- Provide educational information to patients about health center services and help connect them to all services.
- Assist with reporting outcomes related to grants and quality programs.
- May assist with community outreach activities and events to promote access to services.
Minimum Qualifications:
- COVID-19/Flu considerations: Must be COVID and Flu Vaccinated, willing to become vaccinated, or willing to wear a mask if on-site during Upper Respiratory Season (approximately Oct. 1st through Mar 1st).
- Bilingual in Spanish and English preferred. Prefer experience working in community work. This can include advocacy, volunteering, philanthropy, etc.
- A bachelor's degree in social work or a related field is preferred, with professional experience and education considered.
