This position includes a sign-on bonus - LMSW $2,000!JOB SUMMARYUnder the direction of a Licensed Masters Social Worker, the employee is responsible for providing therapeutic counseling services in a school setting. Services are specifically designed for children and adolescents ages 3-21 and their families that are aimed at achieving the best possible social and emotional health status. This position requires the provision of individual and group therapy by using evidence-based curricula and interventions. Works collaboratively with the clinical teams in our area school-based health centers to provide medical referrals for health services and collaboration as needed. Participates in behavioral health activities related to Quality Improvement and other special GLBHC and school-based related projects. Participates in a team-based approach to care in the Patient Centered Medical Home (PCMH). Willing to be reassigned to other school-based behavioral health sites/ locations during times when school is not in session.ESSENTIAL JOB DUTIESReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Accepts referrals from PCP, GLBHC care team, and school officials regarding patients' behavioral health needs. Responds in a timely manner and in a form of communication that is collaborative and reaches the care team effectively. Provides and documents referrals as needed to appropriate providers/resources both within and outside GLBHC. (10%)
  • Provides comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, and/or substance use needs, and/or social determinates of health needs. When assessment indicates, recommends referral to a different specialty provider, other member of the care team, and/or consultation. (30%)
  • Assists in creation of treatment planning by utilizing evidence-based interventions to address SMART goals, objectives, and interventions. Provides individual, family, and/or group therapy using evidence-based practices and interventions to assist patients with psychological, social, emotional, and environmental needs. (15%)
  • Assist in the detection of at risk patient to prevent further psychological or physical deterioration. If needed and/or required use available screening tools, psychological stressors assessment and diagnostic codes in conducting initial screening to determine level of risk and/or determine if referral to another agency is needed. (10%)
  • Maintains pertinent, accurate, and timely documentation. Consults and collaborates with care team staff and patient to address recommended interventions. (10%)
  • Engages with school social worker, other school officials, and care team staff to identify community agencies that provide additional support for educational, social, and health services related to school-based health care. (5%)
  • Provides building level promotion, staff trainings, and professional development such as school climate initiatives, bullying prevention, and suicide prevention programs, etc. (10%)
  • Attends and participates in faculty, staff, and community meetings. Attends School-Based and Behavioral health staff meetings and case consults to provide mental health expertise. Attends and participates in afterhours events to support School-based health initiatives. (5%)
  • Collects data and other relevant information for project evaluation while maintaining cooperative and consultative relationships with psychiatrists, mid-level providers, therapy providers, and other major stakeholders for grant reporting purposes. Meets standards for quality and utilization and follows GLBHC policies, procedures, and expectations. (5%)

  • Serves as a patient advocate to ensure students and their families receive appropriate resources that improve their emotional status.

  • JOB SPECIFICATIONS
  • Education Master's degree in social work from an accredited graduate school.
  • Licensure Licensed or Limited Licensed Master's Social Worker in the State of Michigan. Basic Life Support (BLS) required.
  • Experience Preferred minimum of three (3) years of experience in a behavioral health setting. If Limited Licensed Master's Social Worker, must obtain license in a timely manner and be actively pursuing. Be in good standing and continue to be in good standing with expectations regarding licensure.
  • Skills: Skilled in written and oral communication. Able to demonstrate positive customer service skills. Basic computer skills and ability to work in an electronic medical record.
  • Interpersonal Skills Able to prioritize activities on a short and long-term basis and able to handle multiple tasks efficiently and effectively. Functions effectively in the treatment of patients. Able to resolve interpersonal and professional conflicts appropriately. Able to develop and maintain cooperative and effective working relationships with clinical management, members of the executive structure, Board of Directors, business associates, and community groups.
  • Physical Effort Must be able to sit, stand, and or walk for an entire workday. Must be able to lift, carry, push, pull, and or twist while holding up to 25 lbs. occasionally.
  • Hours of Work Full time; flexible and varied.
  • Travel: Travel may be required within the service area. Use of personal vehicle is possible. Reimbursement commensurate with corporate policy and IRS guidelines.
