Job Summary:
Responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Virtua Hospice for thirteen months following the death of the individual hospice patient. Position will require some evening and/or weekend availability. Requires traveling. Includes community involvement, etc.
Position Responsibilities:
Coordinate the assessment and delivery of grief counseling needs and services (one-to-one, groups and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
During the comprehensive assessment of the patient, an initial bereavement assessment is conducted to the social worker to determine the cultural, social and spiritual factors that may impact the ability of family members or other involved individuals to cope with the patient's death.
Findings from the initial bereavement assessment are incorporated into the patient's plan of care and considered in the bereavement plan of care.
The Bereavement Coordinator is notified of all deaths and initiates the bereavement discussion and care planning at the first IDG meeting following the patient's death.
The Bereavement Coordinator ensures that the bereavement plan of care is followed for 13 months following the patient's death, appropriate to the level of need assessed.
Provide bereavement services and participate in all performance improvement (PI) activities. Bereavement services listed in a patient's bereavement plan of care may include but are not limited to: bereavement visits and counseling, mailings and/or telephone contact.
Maintain a list of bereavement information and referral services in order to service the community and hospice families. Expand and maintain community resource lists as needed.
Provide bereavement support to hospice personnel coping with work-related grief, through one-to-one counseling, supervision of interns providing one-to-one counseling and referrals to community services. Sets up memorials, coordinating and conducting support groups for caregivers of recently deceased patients.
Participate in hospice activities such as in-service education, hospice personnel meetings, staff meetings and relevant committees. Seek and accept guidance from other members of the interdisciplinary group, or from members of the community. Report all bereavement activities to supervisor or organization administrator.
Annually prepare a strategic plan for expansion of the Bereavement Program. Evaluate services to see if they are effective, including coordinating support groups in emotional services.
Complete other duties as assigned and demonstrate flexibility with job responsibilities in all areas. Adheres to Organizational competencies and standards of behavior.
Position Qualifications Required:
Required Experience:
Minimum 2-3 years experience in hospice care.
Required Education:
Social Worker (LSW, MSW, LCSW). Masters required.
Training / Certification / Licensure:
Current valid Social Worker License (LSW) required.
