Job Title/Position: Bereavement Coordinator Reports To: AdministratorJOB DESCRIPTION SUMMARY The Bereavement Coordinator is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Guiding Light Hospice for up to (13) thirteen monthsfollowing the death of the individual hospice patient. The Bereavement Coordinator provides support to families, caregivers, and loved ones of hospice patients before and after the patient's death. This role is essential in helping individuals navigate grief and loss, ensuring they receive compassionate care during a challenging time. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. Conduct assessments to identify the bereavement needs of families and caregivers.2. Develop personalized bereavement care plans tailored to the unique needs of each individual or family.3. Coordinates the assessment and delivery of grief counseling needs and services (one-toone, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.2. Plans for Bereavement Program development, expansion, and refinement through annual evaluation of services.3. Provides bereavement support to hospice personnel coping with work-related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.4. Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.5. Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats and weekly support groups).6. Organize memorial services, support groups, or other activities to honor loved ones and promote healing.6. Functions independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community.7. Maintain accurate records of all bereavement activities and interactions.8. Ensure compliance with federal, state, and organizational policies regarding bereavement care.9. Participates in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.10. Advances professional knowledge and skills.11. Actively participates in quality assessment performance improvement teams and activities.12. Accepts and performs other related duties and responsibilities as required. POSITION QUALIFICATIONS 1. Bachelor's degree or equivalent theological degree from college or university. Experience with families/caregivers and patients through death/dying issues - OR - Master's degree in Social Work with qualifications of experienced clinical grief counseling.2. Experience in grief counseling, hospice, or palliative care.3. Strong interpersonal and communication skills.4. Ability to work independently and as part of a multidisciplinary team.5. Proficiency in electronic medical records (EMR) systems is a plus.PHYSICAL REQUIREMENTS:1. Ability to sit, stand, and move for extended periods.2. Must have reliable transportation to visit clients and attend meetings.Work Environment:1. Office, home, and community settings.2.
