Bereavement Coordinator, VNA of Cape Cod - Cape Cod Healthcare : Job Details

1. Manage the bereavement services programs utilizing professional staff and volunteers as appropriate to the service or event.

2. Adheres to standards of social work practice as set forth by the National Association of social workers and demonstrates an understanding of the hospice philosophy of care.

3. Oversee adequacy and appropriateness of bereavement programs for family members.

4. Develop new bereavement programs and services as needed.

5. Develop educational programs and materials for patients/families, staff and the community on loss, grief and coping with bereavement.

6. Assist agency in educational training program.

7. Design materials for distribution to families eligible for and/or receiving Hospice bereavement services.

8. Plan, implement and supervise bereavement group events.

9. Supervise support staff involved in bereavement program.

10. Oversee bereavement follow-up by patient care staff and volunteers.

11. Oversee and supervise the volunteer component of the bereavement services.

12. Conduct the bereavement section of the interdisciplinary team conference.

13. Participate in the maintenance of the bereavement component of the community and staff and resource libraries.

14. Participate in the orientation and training of new employees and volunteers working in the Hospice program.

15. Prepare service reports as requested by management.

16. With the Chaplain, plan and conduct Memorial/Remembrance Services for the staff and family.

17. Other duties as assigned.

18. Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers.

19. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence.

* Bachelor's degree in social work or Bachelor's degree in seminary, theology, pastoral ministry required

* Demonstrate experience in the areas of life threatening and chronic illness, grief and loss, individual, family and group therapy.

* Experience and interest in program development. Experience in written and oral communication, supervision of staff, documentation requirements and program management

* One (1) year of Social Work experience, preferably in healthcare preferred

* Master's degree preferred
