We are currently seeking an experienced:

C-SNP PATIENT CARE COORDINATOR - LIBERTY ADVANTAGE

Full Time

JOB SUMMARY:

  • Providing care for assigned patients in collaboration with RN case manager and nurse practitioner.
  • Obtains information for assigned patients based on the model of care requirements.
  • Obtains information to assist with evaluation of patient conditions/needs.
  • Communicate effectively with any transition of care.
  • Documents patient information in the electronic medical record.
  • Reviews appointments, labs and diagnostic tests communicating pertinent results to RN Case Manager and nurse practitioner for additional interventions
  • Collaborate with members of IDT for coordination of care
  • Communicate with member and/or family on quarterly basis.
  • Strong communication skills and focus on customer needs and services.
  • Assists with any patient emergency as needed.
  • Recommends and/or conducts specific areas of in-service or continuing education with patients and caregiver as appropriate.
  • Assists with compliance of quality measures for assigned members
  • Participates with ICT meetings for assigned members
  • Performs other duties as assigned.

JOB REQUIREMENTS:

  • Highschool diploma or equivalent education
  • Graduated from an accredited Certified Nursing Assistant (CNA) program
  • Minimum 2 years clinical experience as a Certified Nursing Assistant (CNA) in geriatric, adult or family practice setting
  • Current BLS certification
  • Knowledge of community resources
  • Previous experience working with behavioral health populations
  • Experience working with low income and disadvantaged populations
  • Previous work with vulnerable adults or senior population
  • Must meet company driving standards
  • Must have valid state driver's license and be able to drive a car
  • Must meet company's automobile liability insurance standards and provide proof of coverage
  • Must be able to independently travel to assigned locations

  • Must be able to demonstrate computer skills, including use of e-mail and Microsoft office word and excel
  • Must be able to demonstrate effective written communication skills including but not limited to writing reports and correspondence
  • Must be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families and staff and listening to members, providers and families
  • Demonstrated ability to meet deadlines, and use time effectively and efficiently.
  • Demonstrated successful experience working in an environment of change. Effectively copes with change and uncertainty; responds and shifts gears comfortably
  • Dedicated to meeting the expectations and requirements of both external and internal customers, acts with customers in mind
  • Demonstrated ability to establish and maintain effective relationships with customers, and gain their trust and respect
  • Uses logical methods to solve problems with effective solutions; looks beyond the obvious for solutions
  • Action oriented and sees challenges as opportunities
  • Demonstrated ability to collect information such as vital signs, weights, list of medications, etc.
  • Demonstrated ability to recognize conditions, characteristics, and behaviors that place seniors at risk
  • Pertinent Clinical Work Experience relevant to the population being served.
