Job Description: Join Our Community!\n \n ANCC Magnet Recognized UPMC Passavant is currently hiring a Full - Time Cardiopulmonary Rehab Nurse to support the Cardiac Rehab unit at the McCandless and Cranberry location.\n Responsibilities:\n\nIncorporates national professional organization as well as business unit and health system's goals to improve patient safety, quality and satisfaction\nCreates a caring and compassionate patient focused experience by building healing relationships with patients, families and colleagues.\nAssess condition of patient, takes vitals as necessary watches for change in condition or reactions.\nObserves and reports changes and or significant incidents to physician and/or manager.\nEvaluates status of patients and offers support to patient and family.\nEducates patients and families on treatment plan.\nComplete evaluation and assessment of patient with accurate and adequate documentation\nDevelopment and documentation of individualized patient treatment plan, accurate with respect to patient's age and medical history, its implementation and monitoring of progress and modifications as indicated.\nDevelops inpatient discharge plan and/or outpatient discharge plan.\nPrepares for and responds to emergencies in a manner consistent with department guidelines\nPractices solid communication skills and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and unit practices.\nPatient documentation is comprehensive and promotes communication between caregivers.\nDemonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care.\nActively participates in unit-based shared governance, goal setting and supports the change and transition process to improve quality of care and the practice environment.\nServes as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership.\nParticipates in work that improves patient care and the professional practice environment.\nAdapts to change and demonstrates flexibility with the change process.\nDevelops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation and commitment to patients and colleagues.\nDemonstrates an understanding of cultural differences and holds peers accountable for healthy relationships.\nMaintains a conscious balance between work and personal life.\nModels safe work hours, time management and healthy lifestyle.\nCommunicates with peers and management any safety hazards identified in the workplace.\nConducts, organizes and participates in meetings, in services and continuing education upon request by manager\nAssists with staff development, special projects and community programs\nParticipates in the Co-Worker Feedback program if asked\nDemonstrates knowledge of adult learning principles (and/or teaching children if applicable) and applies in teaching of patients, families, students and new staff.\nProvides detailed and appropriate teaching to patients and families to effectively guide them through the episode of care as well as transition to another level of care and/or home.\nSupports the development of students, new staff and colleagues, may serve as a preceptor.\nCreates an environment of open dialogue, inquiry and continuous development by asking for feedback and improving practice.\nPlays an active role in developing, implementing and gathering outcome data for the annual quality improvement initiative.\nIdentifies opportunity for quality improvement to colleagues and management.\nTakes personal responsibility in improving patient satisfaction with the quality of care and service.\nUtilizes research and evidence-based practice to support improvement in clinical care\nIdentifies research issues or articles related to clinical specialty or areas of interest, discusses opportunities for quality improvement at multidisciplinary rounds, and influences patients' plans of care.\n\nGraduate of an approved school for registered nurses\nMinimum of 3 years nursing experience.\nLicense or eligible for State Board ACSMLicensure, Certifications, and Clearances:\nRegistered Nurse (RN)\nBasic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)\nAct 34\n\n *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
