General DescriptionOneFifteen is a not-for-profit healthcare ecosystem dedicated to the full and sustained recovery of people suffering from drug addiction. OneFifteen has developed a tech-enabled system of care that offers a treatment center, rehabilitation housing, and wrap-around services, all on one state-of-the-art campus. OneFifteen deploys a learning healthcare system to advance our understanding of addiction medicine and the best strategies to promote sustained recovery. Our ultimate goal is to support individuals to live healthy, fulfilling lives free of addiction. As such, we will focus on a patient-centered experience and ensure a patient's whole health needs are fully met, including biopsychosocial needs.The Care Advocate will collaborate with other team members to provide best-practice care with an innovative spirit and help drive a holistic approach to care and reduce stigma. At the same time, the Care Advocate will be comfortable working in ambiguous situations and blend best-in-class care delivery models with advanced technology solutions to engage care team members, improve outcomes, and reduce costs.The Care Advocate is a frontline worker who is a trusted member and has extensive knowledge of the community served and extensive knowledge of community resources to service at-risk populations. The position acts as a liaison between community resources and the participant and/or family to improve service delivery quality and cultural competence. The position will work directly with individuals and family members referred to the program. The Care Advocate coordinates a multi-disciplinary approach to service delivery with treatment partners, community resources, and social service agencies and ensures compliance and fidelity to the Pathways Hub Model. The Care Advocate also provides outreach to at-risk populations and conducts community education on the topics of substance use and abuse and mental health; schedules regular meetings with participants and partner agencies to maintain communication and collaboration about services; works directly with all program members to maintain and retain project data and materials as required; supports the completion and submission of progress and other required reports; serves as a member of local coalitions; develops and maintains strong relationships with stakeholders and community partners, and performs other job-related duties as assigned. Duties and Responsibilities* Responsible for establishing trusting relationships with participants and their families while providing navigation and linkage to needed community resources.* Provide referrals for services to community agencies as appropriate.* Provide ongoing follow-up, basic motivational interviewing and goal setting with participants and family members.* Follow-up with participants via home visits, telecommunication and visits in other settings where participants can be found.* Help participants set personal goals and ensure direct access to resources to support goal attainment.* Help participants connect with transportation resources and provide appointment reminders in special circumstances* Exhibit excellent working relationships with participants, family members and all community partners.* Responsible for providing consistent communication with the participant, family members, and other service providers, and reports clearly describe progress.* Act as a liaison between the participant and family members and community service agencies.* Provide timely written documentation as required.* Work cohesively with all team members.* Attend regular staff meetings, training, and partner and community meetings as required.* Manage an assigned caseload.* Establish trusted advisor relationships with customers and OneFifteen users, have a firm grasp of the details of their accounts, and drive adoption and utilization of OneFifteen solutions.* Travel is required to locations within Montgomery County. Some travel may be required outside of the county.* Drive participants to and from appointments or work, if applicable* Legal and Finance Support (2%)* Work closely with the Supervisor to generate, extract and organize monthly financial reports related to grant activities.* Other duties as assigned.Knowledge and Skills* Extensive knowledge about community resources appropriate to the needs of participants served and family members.* Ability to work with many diverse people, including family members.* Experience working with individuals with mental health and substance use disorders.* Demonstrates cultural competency.* Ability to make suggestions on workflow or system efficiency and effectiveness.* Ability to work independently and be self-directed and flexible.* Ability to perform functions with minimal supervision.* Ability to work at a high-volume level of accuracy.* Demonstrated knowledge of technology, with a preference towards comfort with healthcare software (e.g., EHRs, telehealth software, and mobile applications).* Excellent verbal and written communication skills and the ability to simplify complex concepts for varying levels of audiences.* Excellent customer service and people skills and ability to communicate effectively in written format and oral presentations.* Excellent organizational skills and very detail-oriented.* Experience managing relationships with multiple internal and external stakeholders and partner agencies.* A strong desire to work in a fast-paced, evolving startup environment.* Successfully completed background check, fingerprinting and drug testing.* Satisfactory driving record with no more than 2 moving violations within the previous 3 years.Education and ExperienceMinimum:* Associate's Degree in social services, nursing or related field.* Completion of the Community Health Worker Certification by way of the Ohio Board of Nursing within the first year of employment. Certification must be maintained and recertification is required every two years.Preferred:* Bachelor's Degree in social work, counseling, public health, nursing, or related field.* Licensure in the State of Ohio CDCA, LCDC, LSW, LPC or LPN* Community Health Worker certification by way of the Ohio Board of Nursing Board.Work Conditions and Physical Requirements* Work Setting: Indoors, environmentally controlled* Body Positioning: Spend time sitting and standing, making repetitive motions, spend time using hands to handle, control or feel objects, tools or controls.
