We are looking for a full-time friendly, people-oriented individual who enjoys patient interaction. Ryan White Benefits Eligibility is responsible for providing evaluation, assessment, education, individualized guidance, and follow-up assistance to patients who qualify for ongoing or intermittent public and/or private healthcare benefits.RESPONSIBILITIES- Includes but not limited to the following:1. Provide walk-in assistance and services navigation to unscheduled patients with inquiries regarding the WNCCHS sliding fee discount program, emergency funds assistance and insurance eligibility.2. Assess patients' need for financial assistance by completing WNCCHS Sliding fee discount programs and Ryan White application/attestation annually (e.g., completing intake/update forms) Maintaining case load of patients as assigned. Including, daily courtesy calls to patients as a reminder to provide proof of income.3. Provide excellent customer service by way of patient-specific education and counseling regarding health care financial assistance programs available through the public sector, specifically:* Medicare* Medicaid* Children's Health Insurance Program (CHIP)* Qualified Health Plans (QHP) available through the Health Insurance Marketplace of the Affordable Care Act* Federally Qualified Health Center Program* Ryan White Program* NC HIV Medication Assistance Program (HMAP) including the following sub-programs: Uninsured Medication Assistance Program (UMAP), State Pharmaceutical Assistance Program (SPAP), >Insurance Copayment Assistance Program (ICAP), or Premium & Copayment Assistance Program (PCAP).4. For enrollment activities for patients concerning pharmaceutical assistance, document informed consent for each enrollment in the patient chart.5. Assist patients with obtaining the documentation necessary to complete applications, and helping patients maintain enrollment in eligible assistance programs.6. Help patients complete and maintain enrollment in public health care financial assistance programs. (This includes helping the patient complete the application form, gather the necessary documents, and follow the process through to the final outcome.)7. Maintain required Certified Application Counselor (CAC) credentials to assist patients not enrolled in Ryan White with applying for insurance through the federal healthcare exchange.8. Help patients resolve significant debts by assisting with Emergency Financial Assistance (EFA) applications and/or Copay and Coinsurance Deductible Assistance (CCDA) applications.9. As appropriate assist with connecting patient to care management transportation services.10. Coordinate with Care Management Community Health Worker(s) and others to maintain engagement in care and complete transfer of care when services are terminated.11. Document all services provided in a timely manner.12. Participate in individual and group outreach activities.13. Provide consistently professional communication and interaction with providers, staff, patients, and peers.14. Maintain subject matter expertise in Ryan White services eligibility and requirements.15. Reflect the values of WNCCHS, while following staffing and company policies.16. Know, understand, incorporate, and demonstrate the Mission, Vision, and Values of WNCCHS in behavior, practice, and decision-making.17. Participate in professional development activities.18. Maintain strict confidentiality and adhere to all HIPAA regulations.19. Participate in mandatory in-services, drills, staff meetings and other trainings.20. Perform duties in a prioritized, organized, and orderly manner to maximize efficiency and productivity.21. Facilitate the maintenance of a clean, neat, and well-organized work area.22. Participate in quality improvement activities by initiating or contributing to monitoring, measuring, analyzing, improving and/or controlling program goals, objectives and/or services.23. Support current incentive, regulatory and certification requirements through documentation, participation in initiatives and other activities as directed.24.
