The Care Coordinator Assistant supports the leadership and direction provided by the Care Coordinator and supports projects for the department in conjunction with other leaders. Required Criteria

• Current licensure and registration as a Licensed Practical Nurse in New York State
• Five years clinical experience
• Basic computer skills
