The Mental Health Association of Columbia-Greene Counties Inc. is looking to fill the position of Care Coordinator. This position is full-time, 40 hours per week with a work schedule of Monday through Friday 8:30 am - 4:30pm, $20.879 per hour. The main worksite is in Catskill NY, we also have an office located in Hudson, NY. Benefits include Medical, Dental, Life and AD&D plan. Care Coordination provides services to both Columbia and Greene counties. Care Coordinators provide coordination of behavioral and/or physical health care for any person who is Medicaid eligible, regardless of diagnosis. Care Coordinators can help those who qualify attain the best possible services for physical and/or mental health. Care Coordinators are contracted through our Health Home (Hudson River Health Care) to provide these services. Individualized wellness plans in coordination with other providers' plans are developed to help individuals manage a healthier day-to-day life, with a focus on rehabilitation, recovery, and clinical service linkage. Job duties include:

  • Coordinating with service providers and health plans to secure necessary care for members.
  • Link/refer clients to needed services to support care plan/treatment goals, including medical/behavioral health care, patient education, and self-help recovery and self-management.
  • Conduct case reviews with interdisciplinary team to monitor/evaluate client status/needs.
  • Advocate for services and assist with scheduling of needed services.
  • Coordinate with treating clinicians to assure that services are provided and to assure changes in medical treatment or medical conditions are addressed.
  • Monitor and support clients in making scheduled medical/behavioral/social service appointments.
  • Crisis intervention.
  • Revision of care plan/goals as needed.
**This position offers some flexibility to work remotely based on program needs and supervisor approval** Requirements
  • Must have a bachelor's degree in a social service or counseling discipline such as social work, psychology, nursing, rehabilitation, community mental health social work, psychology, or related field OR Registered Nurse with bachelor's degree and at least 2 years' experience in providing direct services in the mental health field or in linking clients to a broad range of services in the community. Exceptions to the bachelor's degree can be made at the discretion of the program supervisor depending on experience.
  • Experience in providing direct service in the mental health or social service field and linking clients to services in the community is necessary.
  • Proficiency in, or ability to learn, online technology systems is critical.
  • Ability to work with a diverse population of urban, suburban and rural cultures and with community members from varying socioeconomic backgrounds.
  • Demonstrate a commitment to Diversity, Equity, and Inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias.
  • Must be able to work regularly outside the office and in the field to engage with clients.
  • Excellent oral and written communication skills.
  • Familiarity with Greene County and knowledge of managed care is a plus.
