Amergis Staffing is hiring for a Care Coordinator who has experience assisting members with disabilities. They will work in collaboration with the multi-disciplinary team. They will have input in the development of the plan of care for those patients requiring social work intervention. Provides medically necessary intervention to patients who have social or emotional problems which are or are expected to be an impediment to the effective treatment of the patient's medical condition and/or rate of recovery. Location: Chicago, IL Weekly pay + benefits offered! Work Schedule: Monday-Friday only (8am-430pm) License & vehicle required for case visits Responsibilities: Visit members in the home to complete an assessment, develop a care/service plan that meets members needs and provide periodic follow-up Verifies eligibility for wavier services Documents findings in the Care Management System Meets or exceeds timelines associated with case activities Provides succinct presentation of member, and solidifies plan of care with interdisciplinary Care Team Completes a member assessment according to department policy and procedure.
