Supplemental Life may be purchased at group rates.Paid Time Off: Twelve (12) holidays, vacation time (after 6 months) of three (3) weeks per year for the first eight (8) years of service, and sick time.Retirement: Ohio Public Employees Retirement System (OPERS) with 14% annual employer contribution and Deferred Compensation Plans.Job SummaryThe Care Coordinator works with diversion level youth that display behavioral health needs and/or family turmoil. They assess and evaluate the needs of youth while empowering parents to address behavioral health needs redirecting youth away from the juvenile justice system, and in lieu of formal court processing. The Care Coordinator position has the tasks of coordinating services, ensuring engagement via a strength-based case management approach with an emphasis on supporting success with services as opposed to compliance- focused supervision. The Care Coordinator will also assist with completing CANS assessments. Essential Duties and ResponsibilitiesTo perform this job successfully, an individual must be able to satisfactorily perform each essential duty listed below. This position will focus on assessing the needs of these youth and empowering parents to address the needs.Provides services for pre-adjudicated youth and their families in order to reduce recidivism and to minimize involvement with the youth justice system.Develops strength-based and family-focused goals for the purpose of alternative case planning. Maintains case management documents for active youth.Refers youth and/or their families to services and/or programs sponsored by the Court, community agencies and/or other approved providers. Creatively utilizes wraparound supports to link youth to non-traditional supports as an aid to community sustainability.Conducts meetings with families in crisis, establishing safety and community plans. Refers families to appropriate community based service providers, when necessary.Assists with completing the Child And Adolescent Needs and Strengths (CANS) Assessment and maintains certification and yearly training requirements. Serves as a link between treatment agencies and families, Court, and other entities to ensure all parties are equipped with information for resources.Participates in mediation meetings at identified schools to act as a support and collaborator in developing youth educational success planning.Works closely with Court personnel and community partners in a team environment.Assists with training for police and community partners.Performs other duties as assigned.Supervisory ResponsibilitiesHas no direct supervisory responsibility for staff members.Equipment OperatedStandard office equipment and office technology such as copy machines, personal computers and telephones.Confidential DataAll court-related information regarding hearings, pre-adjusdicated cases etc. that the employee may come in contact with by working in the facility.Working ConditionsThis position is subject to varying and unpredictable situations; may handle emergency or crisis situations; is subject to many interruptions; may handle multiple calls and inquiries simultaneously; and may occasionally handle absentee replacement on short notice.Position may be required to travel and visit with clients and family members and have varying working conditions outside of the courthouse. Contact involves occasional exposure to physical injury, illness or hazards as a continuous part of the position. May occasionally have exposure to unsafe working conditions, people, or elements, and may have to take precautions when performing tasks.Potential exposure to hazardous situations that may occur in the court environment. Must be able to tolerate exposure to evidence and testimony that may be disturbing; clients who may potentially be verbally or physically uncooperative; allergens, such as perfumes and dust. Works in a setting that may be subject to quick deliverables, and changing work deadlines.Usual Physical DemandsWhile performing the duties of this job, the employee is required to sit, to speak clearly, and to hear. The employee must occasionally lift and/or move up to 10 pounds.Knowledge, Qualifications, Skills, and AbilitiesTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Demonstrated knowledge of adolescent growth and development, particularly as it relates to substance abuse; knowledge of family dynamics and human behavior; skilled in brokering community resources, identifying and utilizing appropriate resources; effective oral and written communication skills; skilled in case management, interviewing, maintaining good working relationships with a variety of groups; ability to manage stress, time, confidential information while maintaining a professional demeanor. Flexible in work hours to accommodate program needs including, but not limited to, case consultation on an emergency or as needed after work hours, in person or by telephone.Education and/or ExperienceBachelor's Degree in Social Science or related field.ANDThree (3) years' experience with adolescents with co-occurring disorders, five years preferred.Licensure or Certification RequirementsMust possess a valid Ohio driver's license and maintain valid automobile insurance, with a clean driving record and ready access to a safe functioning automobile.Any position which requires operating a motor vehicle as an essential duty of responsibility of the position will be subject to a driving record check through the Ohio Bureau of Motor Vehicles.
