Care Coordinator - The Judge Group : Job Details

We are currently looking for Hybrid Care Coordinators!

This is a full-time, 1 year contract-to-perm role, contracting running through October 2025!

Equipment will be provided, work schedule is Mon-Fri 8am-5pm

In this role, you will assist the responsible RN with telephonic or face-to-face assessments for the identification, evaluation, coordination and management of member's needs, including physical health, behavioral health, social services and long term services and supports.

Simple Requirements:

• LPN or LCSW/LSW/LMSW
• Experience in hospital setting
• Experience in Case Management
