A member of the Nexus family treatment programs, Nexus-Onarga Family Healing is located approximately 90 miles south of Chicago and only a few miles off of I-57 on a historic site in Onarga, IL. Nexus-Onarga provides boys ages 10 to 20 years old a structured, homelike environment where they can safely address their unhealthy sexual behaviors and their emotional and behavioral issues.At Nexus, we believe in changing the course of a child's life by stabilizing families and strengthening mental health through trauma-informed care. Nexus is a growing, innovative organization focused on transforming our level of impact through a heightened focus on family expertise, healing complex trauma, and supporting high-stress youth and families.The Care Coordinator is responsible for participating with the therapeutic team while supporting and reinforcing treatment services by providing guidance and direction to assigned individuals, families, and groups, and coordinating internal and external resources to ensure ongoing therapeutic services and community support is delivered for complete treatment objectives for stabilization achievement.Required Education and Licensure* Bachelor's degree in a clinical discipline (psychology, social work, etc.).* In lieu of clinical degree, any bachelor's degree plus 2 years of related experience working with at risk youth required.* In lieu of bachelor's degree, will consider MHP with 5 years of related experience working with at risk youth.* Meets the Mental Health Service Definitions and Professional Qualifications of a Mental Health Professional (MHP)* Minimum 21 years of age.* Valid driver's license required. Must meet state regulating agency and corporate driving requirements.Schedule & Salary:* Schedule:* Tuesday-Saturday or Sunday-Thursday* Five, eight-hour days a week with availability to meet the needs of youth & families* Salary: Starting at $24-$26 per hour based on prior years of relatable experience & credentialsPrimary responsibilities include:* Responsible for providing mental health services/counseling services to assigned youth and documenting their service delivery. Service provision will meet or expected minimum standard.* Manages and coordinates resources and activities for each assigned client(s) to ensure that the unique treatment needs, and requirements of each individual are met.* Participates and facilitates the client and family's linkage to the therapeutic treatment process and activities, identifies barriers and connects with support throughout the treatment process to support to post transition or discharge placement.* Works closely with the teams to create supported engagement, guidance plans, stabilization plans, treatment plans, therapeutic interventions, and discharge plans.* Participates as an active member of an interdisciplinary team to develop treatment plans and completes other client or case documents as needed.* Provides assistance for families regarding locating resources available to the families and client that will support the healthy functioning of the family unit.* Works to bridge relationships with clients, adults, and families, with outside organizations and provides role modeling for advocating for needs.* Participates in and/or facilitates all required meetings and conferences relative to assigned clients.* Exhibits clear, professional, respectful, and effective communication when interfacing with clients, coworkers, families, and external contacts and willingly share knowledge, information and resources appropriately.* Maintains and completes paperwork and compiles documentation required by the organization and for the governing/accrediting agencies; maintain current files on all individuals enrolled in services.* Works effectively with internal stakeholders to successfully implement plans for achievement of treatment objectives and community reintegration.* Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients.Work Environment:* Must be able to lift to 50 pounds unassisted and up to 100+ pounds with assistance.* Must be able to stand and walk for extended periods of time (2-4 hours).* Must be able to twist, bend and stretch in a manner conducive to the execution of daily activities within the residential program.* Must be able to assist in proper restraining of clients as needed.* This role routinely uses standard office equipment such as desktop and/or laptop computers, photocopiers, and smartphones.At Nexus Family Healing, our voices and actions are focused on recognizing, affirming and respecting people of every race, ethnic background, socio-economic status, sexual orientation, gender expression and faith.Our ICARE Values:* Innovation: Leading the way and implementing creative, cutting-edge ideas and approaches* Compassion: Listening, honoring differences, and showing respect, kindness, empathy care, and concern* Agility: Exhibiting flexibility and adapting quickly* Responsiveness: Being quick, positive, and accurate* Excellence: Demonstrating quality results that surpass ordinary standardsAPPLY TODAY TO BE CONTACTED BY OUR RECRUITING TEAM!
