This position reports directly to the Care Coordinator Supervisor.Responsibilities* Determines eligibility and creates care plans for Illinois Department on Aging* Performs assessments and/or reassessments of senior citizens within their own homes, area hospitals, or long term care facilities as authorized by SHSDC and in accordance with the approved policies and procedures* Determines, coordinates, and develops written care plans related to results of the assessment* Coordinates services for older adults to remain independent* Educates on resources, options and provide case management to older adults and their families* Assists and completes Medicaid applications and documentation of DPA eligibility for appropriate CCC clients under the guidelines and policies for IDOA and IDPA.Requirements* Requires a BA/BS in the human service field with emphasis on social work, rehabilitation counseling, or psychology preferred* Must have valid driver's license and transportationSalary Range -$43,888.00- $46,561Full-time employee benefits include:* 12 vacation days earned in the first year* 11 paid personal days/holidays per calendar year* 6 sick days earned per year* Retirement plan with 3% company contribution after becoming eligible in the plan (a matching contribution by the employee is not required to receive this benefit).
