Care Coordinator - Hospice of Darke County : Job Details

Summary of Position: The Care Coordinator supports the social work team in providing comprehensive social services to patients and their families. This role involves assisting with coordinating care and ensuring that patients receive the necessary support and resources to improve their health and well-being.Education and Experience:

Associates degree social work, psychology, or a related field. Licensed SW Assistant in the state of Ohio highly preferred.

Minimum of one year of experience and knowledge of Microsoft Office products.

Previous experience in healthcare or social services setting is highly preferred.

Special Skills and Requirements:

Shall have a valid driver's license.

Excellent customer service skills.

Detail oriented with excellent communication, documentation, and organizational skills.

Ability to follow instructions.

Ability to work independently without supervision.

Travel to other sites as needed. Travel may be in inclement weather.

Ability to lift 25 lbs. of supplies or equipment.

Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
