Care Coordinator - LPC / LCSW / LPCC / CSW - Behavioral Health Boulder Community Health, Boulder, CO 80303 Full-Time Days, 40 hours per week $31.58 - $47.38 an hour - LPC / LCSW $28.65 - $42.97 an hour - LPCC / CSW $2.50 for weekend shifts. Boulder Community Health is seeking a Care Coordinator LPC or LCSW that is responsible for providing psychotherapy and care management services. The position requires the ability to promote the coordination, collaboration and continuity of care from admission through discharge and post-discharge. The position necessitates leadership skills with the ability to direct and evaluate all aspects of patient care, and to assure that care is delivered a timely, effective and efficient manner.
