Job Description :
The Care Coordinator position provides case management for an assigned caseload of clients under PeopleCare Health Services. The primary goal of Case Management is to ensure the delivery of appropriate services based on the client's care plan and that services are being provided properly and with quality by assigned care staff. In addition, case management includes coordination of care with clients SEP, family, medical personnel, and any other agencies providing services. Duties and Responsibilitiesa. Case Loads: Maintain assigned evolving caseload.b. 90 Day Visits/ CSRs: In coordination with Agency Manager and program team members, schedule, complete, prep, and document state-required 90-day visits and/or continued stay reviews (CSRs)c.
