Care CoordinatorIn this role, you will collaborate with care teams to provide high-quality, patient-centered care while ensuring continuity and addressing care gaps for high-risk patients.Key Responsibilities:Patient Outreach: Engage high-risk, post-discharge patients and schedule follow-up appointments with PCPs.Care Coordination: Work with care teams to manage patient records and track care gaps.Community Support: Connect patients with local resources to address their physical and psychosocial needs.Team Collaboration: Assist nursing, social work, and multi-disciplinary teams to enhance patient outcomes.Qualifications:1- 3 years in a medical office or healthcare setting.Experience with Care CoordinationPreferred: 2 years as an LPN or MA or Pharmacy TechTechnical Skills: Proficient with Microsoft Office and Electronic Health Records (EHR).Skills for Success:Strong communication and collaboration abilities.Ability to work independently and manage multiple tasks.Flexibility and adaptability in a dynamic healthcare environment.Cultural sensitivity and effective engagement with diverse populations.Training & Meetings:On-Site Training: 3 weeks of training on-site.Occasional On-Site Meetings: Periodic meetings will be held on-site.Shift:8-4pmMonday-FridayRemote: 100% remote work outside of training and limited on-site meetingsFor California Applicants: We will consider for employment all qualified Applicants, including those with criminal histories, in a manner consistent with the requirements of applicable federal, state and local laws, including the City of Los Angeles' Fair Chance Initiative for Hiring Ordinance (FCIHO), Los Angeles Fair Chance Ordinance for Employers (ULAC), The San Francisco Fair Chance Ordinance (FCO) , and the California Fair Chance Act (CFCA).
