TITLE: Care Coordinator (Wabasha/Red Wing) PROGRAM: Community Based Care Coordination JOB SUMMARY: Hiawatha Valley Mental Health Center is seeking a Care Coordinator responsible for coordinating integrated medical and behavioral health care for consumers. â€The care coordinator arranges regular meetings of the multidisciplinary treatment team, enlists consumers' participation on the team, coordinates the completion of an integrated treatment plan for each consumer, monitors symptoms and response to medical and behavioral health treatments using structured instruments and consumer interviews, works closely with the primary care and behavioral health care providers to monitor consumer outcomes and revise the treatment plan as necessary to ensure the appropriate level of care is being provided through a person-centered system of care. â€The Care Coordinator organizes consumer care activities to meet the consumer's medical and behavioral health care as well as social services, housing, educational systems and employment opportunities as necessary to facilitate wellness and recovery of the whole person. The Care Coordinator also shares information among all of the participants concerned with a consumer's care to achieve safer and more effective care. JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:

  • Monitors consumer compliance with Certified Community Behavioral Health Center (CCBHC) data outcome measures.
  • Consults with consumer and multi-disciplinaryâ€treatment team about treatment options and preferences;
  • Care Coordinator initiates and monitors progress of person-centered or family centered integrated treatment plan for all CCBHC consumers.
  • Coordinates care (routine, urgent, crisis) among primary healthcare, behavioral healthcare, dental, pharmacy, and specialty services inside and outside the agency, including, but not limited to:
  • Scheduling and monitoring appointments
  • Scheduling follow-up services and post-hospitalization appointments
  • Lab work
  • Prescription adherence
  • Secure releases of information from past or current providers andâ€obtains relevant information
  • Facilitates and monitors internal and external treatment referrals.
  • Monitors progress in achieving goals to demonstrate improvement in the care and clinical outcome of the consumers receiving care coordination.
  • Coordinates and leads regular multi-disciplinary team meetings with all clinicians serving the consumer.
JOB QUALIFICATIONS AND REQUIREMENTS:
  • Must meet minimum qualifications as a Mental Health Practitioner as outlined in MN Statute 245i.04: Sec. 245I.04 MN Statutes
  • Ability to work successfully independently and as part of each consumer's multi-disciplinary team.
  • Ability to work with diverse populations including age, gender, ethnicity, and cultural components of CCBHC consumers
  • Skilled in planning, problem solving and decision-making. Must be highly organized and creative with an ability to locate resources for clients as needed.

  • Must possess a vehicle valid driver's license and a willingness to travel as needed to client or organization locations throughout SE MN.
PHYSICAL REQUIREMENTS FOR POSITION: Must be able to walk, sit, stand, crouch, twist, bend, reach overhead, maneuver stairs and stretch in a manner conducive to the execution of daily activities. Must be able to move around the office and community settings on a daily basis. While performing the duties of this job, the employee must communicate with others and exchange information. Employee regularly operates a computer and other office equipment on a daily basis. Occasional bending and lifting up to 30 pounds.   • Paid Leave Time: Begins accruing at 4.46 hours per paycheck, with 16 hours available upon hire (prorated for PT employees).
