The Hospital of Central Connecticut is an acute-care community teaching hospital, we provide comprehensive inpatient and outpatient services in general medicine and surgery, as well as a wide variety of specialties. The social worker (LMSW) is a graduate level professional that has demonstrated expertise in a healthcare setting in the assessment and treatment of patients along the continuum of care. Develops and implements discharge planning within an interdisciplinary healthcare team, addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post-acute care. Works collaboratively with the providers to identify discharge barriers and delays in order to optimize most efficient use of acute care hospital days and prevent prolonged length of stay. Ability to address all aspects of patient's psychosocial needs including leading family meetings, providing bereavement support and grief counseling, advocacy with community agencies and within the acute care interdisciplinary team and addressing of substance abuse. Works collaboratively with all teams in the hospital to optimize patient's quality of life by addressing patient's needs holistically. Maintains ethical principles and professional standards of practice in adherence to the NASW code of ethics.

Education
Licensure, Certification, Registration
Valid Masters of Social Work License with the State of Connecticut
Preferred bilingual in Spanish/English
Public Health experience preferred
