Job DescriptionPrimary Functions: The care coordinator supports the departments from a team approach in support of families to promote timely access to needed care. Content expert of services within the departments, community services and insurance benefits to properly educate families and referring providers to ensure patients receive the appropriate care. This positions hours are between 7:45am - 5:15pm Essential Functions:
Assist with gathering and tracking necessary documentation for providers to conduct an efficient and meaningful patient evaluation. Assess patient information to determine what services are available within Nemours and community. Collaborate with Lead clinical coordinator, manager, other Nemours colleagues to ensure patients and families receive current and correct service and financial information. Assist in the education to referring providers/agencies regarding available services within supported departments. Extract necessary documentation for new patients visits to ensure an efficient and streamlined appointment for patients aiding to shorter appointment lengths and increasing access to care. Review coverage benefits and financial responsibilities to families supporting awareness of potential out of pocket costs. Connect families to additional resources as needed such as clinical care coordination and social work. Advocate for families and the work of the department. Raise barriers to ensure processes are not impacting or delaying patient care.
