Apply now to see what it means to us and what it's like to work somewhere your voice is heard, your wellness is a priority, and your success matters.JOB TITLE:Care CoordinatorEMPLOYMENT TYPE:Full-time, 40 hours/week SCHEDULED HOURS: Monday-Friday, 8:30am-5pm, one evening a week until 8PMPROGRAM/LOCATION:SBIRT PATHS, East Hartford, CTPC#: 2921ABOUT THE PROGRAM: The Screening, Brief Intervention, & Referral to Treatment Prevention & Access to Treatment Hubs and Services (SBIRT PATHS)The Screening, Brief Intervention, and Referral to Treatment Prevention and Access to Treatment Hubs and Services (SBIRT PATHS) initiative will serve youth ages 12-21 with substance use and/or co-occurring disorders (SUD/COD) to ensure more youth access effective care in a timely way. The SBIRT PATHS initiative will establish a Connecticut (CT) community-based model of co-occurring care for youth that addresses gaps in early substance use (SU) services, prevents the progression of substance use disorders alongside other unmet social needs, and facilitates connections between levels of care. This initiative will expand three evidence-based practices (EBPs) for youth including: Screening, Brief Intervention, and Referral to Treatment (SBIRT), Motivational Enhancement Therapy/Cognitive Behavioral Therapy (MET/CBT), and low-touch Service Coordination using the Wraparound model.Duties & Responsibilities
  • Participate as member of multi-disciplinary treatment team
  • Track and coordinate client intake, services and referrals as applicable
  • Provide assistance to team including coordination of treatment planning and providing daily triage.
  • Educate and link individuals to self-help groups, natural support and other resources as needed.
  • Assist with linking clients with community support systems and agencies and in obtaining benefits for which they may be qualified: insurance, housing, treatment, representative payee, legal services, medical services, education and employment.
  • Maintain careful documentation of contacts and communication to maintain quality services. Complete all required documentation in compliance with agency policy.
  • Work as a liaison between agency and other relevant providers and teams to coordinate care for referred clients.
  • Assist with scheduling and providing transportation to and from appointments.
  • Maintains documentation in compliance with agency and program standards.
  • Participates in staff meetings, supervision, training and other meetings as assigned.
  • Establish knowledge of cultures and communication styles associated with various groups, particularly oppressed groups, to provide culturally sensitive and competent care.
  • Provides crisis intervention and support as clinically indicated.
  • Additional duties as required.
QUALIFICATIONS: Education: Bachelor's degree in human service-related field required. Experience may be accepted in lieu of education for some positionsExperience: A minimum of two years' related experience preferredLicensure/Certification/Registration: Complete any program specific training required.
